This is to request Authority to Travel of Mrs. Rosalina DG. Pangilinan,


Disbursing Officer of Mabuga Elementary School , Division of Gapan City,Gapan
North District to go at DEPED Central Office, DepEd Complex, Meralco Ave, Pasig,
Metro Manila for the purpose of getting the books and pencils donated by Usec.
Tonisito M.C. Umali, Esq.,Undersecretary for Legislative Liaison Office, External
Partnerships Service and School Sports on March 27, 2019. To avoid interruption of
class of Madam Rosalina DG. Pangilinan who was the Grade 5 adviser, we combined
the Grade 5 pupils to Grade 6 pupils under the supervision of the Grade 6 Adviser
Madam Menaida A. Meneses.

Note: Teacher in Charge, Hazel A. Dela Peña will go also with the Schools’ FPTA
President Madam Iraida Manalili. We seek for the help of the Barangay Captain
Honorable Anselmo G. Cabatuando. He gave his permission to use the car (FOTON),
service of Barangay Mabuga, Gapan City and let their driver to accompany and drive
for us
