Our office requested for the travel certificate from your office in lieu of the lost boarding pass of the ff.
names and their dates of travel from Manila to Iloilo and vice versa from period of August 1-4, 2018.

We mentioned the purpose of the request on our previous communication and your office replied that
the travel certification/s we requested is now ready for distribution with a control numbers 18-2065 A-C.

In view of this, we authorized MARY HENZ ANTONETTE Z. BANO, one of our office personnel, to claim
the travel certificate of the abovementioned names.
