What to write on the “Remarks” in
school forms?

 In SF9 (Form 138) & SF10 (Form 137),


PASSED or FAILED

 In SF5 ( Report on Promotion)


PROMOTED /CONDITIONAL LY PROMOTED /RETAINED
Who will give the Speech on behalf of
the completes /graduates?

• In Elementary and Junior High, all students who


got the highest general averages in regular class
and special programs (SSES, STE, SP-ICT,SPA, SPJ
and SPS)

•In Senior High School, only 1 will deliver the


speech regardless of the strand
In CAT Included in the Senior High
School?
 May or may not be included, not graded

 Teachers can included other subjects, eg. CAT, under the


learning areas

 CAT is under MAPEH subject

 CAT is encouraged to be a community service and not a


military training
Can students with line of 7 grades be
included in the honor roll?

• Yes (as long as the student meets the GA of 90%


and passed all the subjects, DepEd Order No. 36,
s. 2016)
Who are promoted? Retained?

• Promoted those who passed all learning areas


• Retained those who failed in 3 or more subjects
• Conditionally promoted those who failed in 1 or 2
subjects
If retained, will the student take all
the subject?

• No, only the failing subjects


How to determine the honors in Grade
12?

 Get the General Average in 4

When to conduct recognition day?
 In small elementary schools, conduct recognition day and
graduation at the same time and date
In big elementary schools, conduct separate dates for
recognition and graduation
 In small secondary schools, conduct a separate
recognition day. Completion and graduation should be
conducted on the same date and time
In big secondary schools, conduct separate dates for
recognition day, completion and graduation ceremony.
Where to write additional subjects(
eg. SP-ICT,SPA,SPS,STE. Etc) in school
form?

• Use the vacant space in SF 9 and SF 10


If there are failing grades, what
documents should be attached by the
advisers/ subject teachers?
• Remedial forms and reports(schedules, list of failing students, lesson plan
for remedial class)
• Students portfolio
• Teachers porfolio
• List of competencies not mastered
• Anecdotal records
• Home visitation report
• Any other documents (class records, attendance, etc)
•All documents should be signed by the immediate superior and principal
Is “Student Profile” required to be
attached to school forms?

• Yes, with attached updated photo and information


• Student Profile should be provided by the Guidance
Coordinator
Should birth certificate be attached to
the school forms?

• Yes, it should be original and complete


NSO or PSA birth certificate pages
Who is authorized to change surnames
due to the acknowledgement of the
father in the school forms?

• Upon the presentation of the documents, the


teachers /adviser should change the new surname
If the learner can’t present or provide
the original NSO or PSA birth
certificate, what should the teachers
do?

• Require the parents of all the students most


especially the parents of the graduating students.
If the learner failed to provide form
137 from his/her previous school
especially from private schools, will
he/she allowed to be promoted in the
next grade level?

• NO, require them to provide the


necessary forms
Should the EPS or monitoring
supervisor attend and confirm in the
graduation ceremony in elementary
schools?

• Yes, the PSDS should provide the


monitoring EPS with the schedules
If the student failed to take remedial
class during summer for his / her 1 or
2 failing subjects, should he/ she take
the subjects during the whole year of
the next SY?

• The student should only take the


unmastered competencies.
If a graduating student have failing
grades from present or previous grade
levels, will he/she be allowed to join
in the graduations ceremony?

• NO, attendance in the graduation ceremony


will also confirm his/her graduation
