STEP HAZARD CAUSE PREVENTIVE MEASURE


1. Reach into right box and select casting Strike hand against rotating grinding wheel Box located inappropriately beneath wheel area Relocate box to side of wheel
2. Grasp casting, list and position Tear hand on corner of casters Corners of caster are sharp and unfinished Correct corner or require use of gloves
3. Push casting against wheel and grind burr Strike hand against rotating grinding wheel Wheel guard is too small for function Correct size of guard
Wheel could explode and injure operator Incorrectly installed grinding wheel Check rpm rating of wheel
Cracked or defective wheel Properly inspect wheel
Flying sparks/chips could injure operator or cause fire Excessive grinding wheel friction with caster Require wearing of protective equipment
Review fire prevention procedures
Ensure machine guards are in place
Respirable dusts could injure operator Dust from caster metal and wheel material Provide local exhaust system to machine
Provide respirator as last resort
Sleeves/loose clothing could get caught in Loose clothing too close to point-of-operation Provide rubber bands to restrain clothing
machine
4. Place finished casting into box Strike hand against casting Inappropriate build-up of completed stock Remove completed stock routinely
