AFFIDAVIT OF LOSS

I, LOURAINE GRACE T. LAURIO, of legal age, Filipino, single and residing at


P-1, Simaya, Malaybalay City, Bukidnon, after having been sworn in accordance with law,
do hereby depose and state:

1. That I am a holder/owner of Student Identification Card issued by Valencia


College Inc.;

2. That on February 21, 2019 the said Student Identification Card was lost;

3. That despite diligent and earnest efforts to retrieve or locate the said Student
Identification Card, I am unable to find the same;

4. That due to this circumstance, the Student Identification Card should now be
considered, for all intents and purposes, lost;

5. That I am executing this affidavit to attest the truth of the foregoing and to
request for the issuance of a new Student Identification Card in my favor.

6. That each and all of the facts herein stated are true and that I am willing to
testify before any court of justice to substantiate the truth of each and all of
the above said facts.

IN WITNESS WHEREOF, I have hereunto signed my name below, this 13th day of
March 2019, at Valencia City, Bukidnon.
