Conduct Competency Assessment First and Foremost
Then, I should (2) plan and prepare the evidence gathering process having
received the assessment package from the TESDA Representative composed of the
following:
1 copy of assessor’s guide
1 copy of candidate’s guide
Competency Assessment Results Summary (CARS)
Rating Sheets corresponding to the number of candidates
(3) Prepare the Candidate
Good morning, everyone! Before we start the orientation, I will give you 5
minutes to attend to your personal needs. The comfort room is located at the back
while our canteen is just outside the assessment center across the building.
However during the assessment, you are not allowed to leave the premises of the
assessment center.

Good morning once again! I am CHRISTIAN CHRISTOPHER D. LOPEZ, a
registered nurse, licensed professional teacher and civil service professional, who
will serve as your accredited competency assessor in CAREGIVING NC II. Welcome
to Saint Vincent’s College, Inc, your assessment center!
Before anything else, I would like to acknowledge the presence of Dr. Joseph
Refugio, our ACAC manager, and Engr. Hermil Contrivida, representative from
TESDA Provincial Office. They are here to oversee the conduct of the Competency
Assessment to check if it is in accordance with the methodology and procedures of
TESDA. Please do not worry as they are just here to observe and not the ones to
assess you.
Before we start, I will check your Attendance first. As I call your name, kindly
present your admission slip and affix your signature on the space provided in the
attendance sheet. *(The assessor compares the signature of the attendance sheet
and the admission slip)
Candidate 1, please sign the attendance.

I have here with me your accomplished Application Form and Self-
Assessment Guide, This indicates that you can perform all units of competency
enumerated in Caregiving NCII.
Again, I am in charge to administer the competency assessment in Caregiving
NC II which has 11 units of competency that are clustered into 3 projects. Project 1
(Provide Care and Support to Infants/Toddlers and Children) covers the Units of
Competency 1, 2, 3, & 4; Project 2 (Provide Care and Support to Elderly and People
with Special Needs) covers Units of Competency 5, 6, 7, & 8; and Project 3
(Performing Housekeeping Activities) covers Units of Competency 9, 10, & 11.
I will collect evidences through Demonstration and Observation with Oral
Questioning & Interview. This assessment method can help measure your
underpinning knowledge and skills.
You are given 3 hours and a half to finish the DEMONSTRATION AND THE
INTERVIEW.
Project 1 – 1 hour
Project 2 - 1 hour and 30 minutes
Project 3 – 1 hour
If 1 of the units of competency is not performed satisfactorily as set by the
standard of the assessment methodology, you will then be re-assessed for the
whole project. You can re-assess anytime whenever you are ready. You are only
given a Certificate of Competency (COC) for the project you have performed and
rated as satisfactory. Therefore, you have to pass or perform satisfactorily all the
11 units of competency under the 3 clustered projects to get the National
Certificate.
If you find it difficult to understand the given instruction, please don’t
hesitate to call my attention because I am willing to explain and translate if for you.
If you have concerns & questions regarding the assessment you may ask me
so I can assist you. But once the assessment started please refrain from asking
questions because it is no longer allowed. If you have any physical difficulties,
please inform me right away so we can address it immediately.
In case you have any question regarding the assessment result you have the
right to appeal, you can submit your appeal to the TESDA Provincial Office where
the assessment centre and the assessor are accredited within 24 hours.
If you have passed the assessment and you are competent, you can claim
your National Certificate at the Provincial Office located at Olingan, Dipolog City,
after 5 working days.
If found not competent- don’t worry about it, you can be reassessed anytime
at your own convenience. In case the reassessment is full qualification you have to
undergo the whole assessment process. If found to be competent in some unit
of competencies the reassessment will just focused on the unit of competencies
that you are found to be NOT YET COMPETENT.
IF you fail 2 consecutive assessments, you are advised to take a refresher
course before applying for reassessment.
Kindly observe safety measures when working with our equipment and tools.
If in case accident occur, please call my attention right away so that proper first aid
will be administered. In case you need a first aid, we have a first aide cabinet at the
right side of the hallway.
With the result of your assessment, we observe strict confidentiality this is
to ensure your integrity, this result is only made available to the right personnel
and to the institution were had your actual training. In relation to this, the
assessment package is placed in the secured place, to guarantee the integrity of the
assessment package. Please do refrain from giving any form of information that
took place in the assessment process.
Lastly, please handle the dummy and other equipment with care.
Any question or clarification? If there is none, I will NOW give you this
COMPETENCY AGREEMENT SHEET. Please read and understand carefully before
signing it. If you need assistance, I am willing to assist you.
Okay, are you ready? Who wants to use the comfort room before we will
start with our assessment? Any clarifications or questions? So, the materials, tools
and equipment are located in the resource area. If you need assistance just tell me.
If you don’t have any questions or clarification, then I am going to give you a
copy of “Specific Instruction”, please read them carefully if you have questions,
please ask it now. I may be able to translate it to your local dialect. If you need
assistance, I will be more than willing to assist you.
Candidates, please proceed to the assessment area so we can begin. Candidates, this is
your Assessment Area. Given the tools, materials & and equipment, you are going to perform

Do you have any questions; you may ask it now. When assessment starts, you can no
longer ask. Please be informed after the demonstration there will be oral questioning.
If you are already done, please call my attention so I can check your work.

ASSESSOR:
Miss Candidate #1, you performed very well in Projects 1 and 3. You
performed satisfactorily in providing infant and adult CPR. I appreciate that
however, I was observing you while doing transfer and ambulation. You were not
able to perform the required standard set in the qualification. I am sorry to tell you
that you have to review/practice the activities related to the competency
requirements of the qualification.
If you are not satisfied with the result of the assessment you may file a
complaint or an appeal to the TESDA provincial office, where your Assessor and
Center are accredited. Do you have any questions?
You come back anytime for re assessment or if you feel confident enough
that you acquired the competence. Don’t worry you will be re assessed for Project
2 only that was found to be NOT YET COMPETENT (NYC).
ASSESSOR:
For candidate #2, you performed the tasks within the standard requirements
of the Caregiving NC II. Congratulations!
Kindly sign here your signature in the candidate’s signature portion.
You can get your National Certificate from the TESDA Provincial Office
located at Barangay Olingan, Dipolog City, within 3 working days based on the date
of your assessment. Just look for Engr. H. Contrivida. He is in charge of the
Certificates. Thank you. Continue to practice your qualification for you not to
forget.
