Shift Handover Policy

1. Scope

The purpose of this document is to provide direction for Operations personnel to ensure that the
handover carried in sufficient clarity. The handover process shall be carried in a formal (both written
and verbal actions) on working site and consistent manner to assure safe and effective operation.

3. Definitions:

Shift handover is vital for all shift crew where people work a variety of shift over the course of a
twenty-four-hours period or other type of shift pattern to ensure effective communication.

5. Process
5.1. Incoming /Outgoing Crew Change personnel should obtain all relevant documentation “face-to
face on site” such as handover reports, near miss incident, onsite hazards, extended work permits,
food safety hazard & incident, log sheets, logbooks, memos, reports, dash board, Radio … etc.
5.2. The procedure shall cover normal rotational or extended crew changes and additionally any works
being carried out over a prolonged period that involve the working of more than one complete
crew, such as maintenance campaigns, well service works etc.
5.3. Handover note / instructions shall be entered on to the appropriate Form and archived for future
reference on completion of the handover. This shall ensure that personnel in key positions shall
be able to reference historical as well as current information regarding work in progress or
completed. The archived information shall be accessible to all personnel. Compliance shall ensure
that the records can be audited.
5.4. For all safety handovers and activities that could affect the safe operation of the facility / station/
people/ environment a “face-to face on site” verbal transfer of information should be conducted
between outgoing and incoming personnel in addition of the above.
5.5. The handover meeting should last sufficiently long enough for all points on the Handover
Information “10-15 Minutes”.

5.6. Conduct an onsite visit if necessary (based on the staff position as some are already on site).
5.7. All handover info to be documented in the shift log.
5.8. Any shift crew identifies an inaccuracy or uncompleted handover is requested to notify the shift
supervisor/operation manager so that appropriate action can be taken
5.9. Log book to be completed & signed by each operator. The Log Books reports are official
documents which will reflect all handing over process
3.10 Shift log to be completed & sent via e-mail by each shift supervisor. The Supervisor shift
report is official document which will reflect all handing over process.
