Acknowledgement
Success is sweet but the secret is ‘sweat of the brow’ – this I
realize while I was in training! Even though it required a lot of
physical and mental effort during the process, it bore a real
sweet fruit. Every moment of my training period was full of
lesson-learning and acquiring experience.

RESEARCH METHODOLOGY
For a clear perception of the term research, one
should know the meaning of scientific method. The
two terms, research and scientific method, are
closely related. Research as we have already
stated, can be termed as “an inquiry into the
nature of, the reasons for, and the consequences
of any particular set of circumstances, whether
these circumstances are experimentally controlled
or recorded just as they occur. Further, research
implies the researcher is interested in more than
particular results; he is interested in the
repeatability of the results in their extentions to
more complicated and general situations”.
Scientific method is the pursuit of truth as
determined by logical considerations. The ideal of
science is to achieve a systematic interrelation of
facts. Scientific method attempts to achieve “this
ideal by experimentation , observation , logical
arguments from accepted postulates and a
combination of these three in varying proportions.”
Experimentation is done to test hypotheses and to
discover new relationship if any, among variables.
But the conclusions drawn on the basis of
experimental data are generally criticized for either
faulty assumption, poorly designed experiments,
badly executed experiments or faulty
interpretations. As such the researcher must pay
all possible attention while developing the
experimental design and must state only probable
inference. The purpose of survey investigations
may also be to provide scientifically gathered
information to work as a basis for the researchers
for their conclusions.

HOSPITAL PROFILE
The AMRI Hospital Ltd., Mukundapur today is a unified Group with 1000
beds, over 500 doctors located in Kolkata, Bengal and Bhubaneswar,
Odissa.

About 10,000 surgeries are performed successfully every year. Its super
speciality tertiary care units are located at Dhakuria, Salt Lake,
Mukundapur and Bhubaneswar along with a state of the art day-care-
facility - AMRI Medical Centre at Southern Avenue, Kolkata.

With a slew of new investments and under a vibrant new


board of Directors, guided by the dynamic Group CEO,
Mr. Rupak Barua, AMRI Group is all set to implement
sweeping changes to head for a super-fast track of growth. Backed by the
finest technologies and a dedicated team of doctors, nurses, technicians and

The Background:

In early 1990s the Government of West Bengal handed over the charge of
‘Niramoy’ - a Government owned Polyclinic at Dhakuria, to Mr. Shravan
Todi of Shrachi Group. Mr. Todi inducted Emami and another promoter, to
build Advanced Medicare & Research Institute Ltd., a super specialty
hospital with sizeable investment to offer quality healthcare. The name was
finally changed to AMRI Hospitals Limited in 2009. The most visible value
that the Emami Group brought to the institution was a spirit of passionate
entrepreneurship. With growing citizen awareness there emerged a greater

QUALITY MANAGEMENT

Location: Basement
Purpose:
 To provide an environment which assures safety for patients/clients,
staff and the public, within a framework of continuously improving
quality of care.
 To promote a quality culture and place quality at the core of service
delivery.
 To encourage attainment of best practice.
 To promote a patient/client-centred organisation and delivery of
service.

Objectives:
 To improve the quality of medical and behavioural health
care and service provided.

 To identify, develop, and/or enhance activities that


promote member safety and encourage a reduction in
medical errors

 To communicate with practitioners/providers of


care/service about quality activities, provide feedback on
results of plan-wide and practice-specific performance
assessments, and collaboratively develop improvement
plans

 To disseminate information on practitioner/provider


performance to promote member empowerment and
informed decision making.

 To document and report the results of monitoring activities,


barrier analyses, recommendations for improvement
activities, and other program activities to the appropriate
committees.

Continuous Quality Improvement (CQI):
Quality improvement is about ensuring that our focus is on
improving, not just maintaining our services at Hospital.
Quality improvement involves a focus on the safety,
effectiveness, efficiency, acceptability, accessibility and
appropriateness of services for consumers (who might be
patients, relatives/parents, or the hospital and other health
care professionals).

Purpose of continuous quality


improvement is to-

Goals of continuous quality


improvement are –
1. To employ the staff-members of wards, departments etc.
of the Hospital continuously to document the work-done
properly so that at any point of time in future, the lapse
can be pointed out and properly addressed.

2. To maintain a Quality improvement team to be


responsible for each key function and evaluate the need
for quality improvement activities for the function on an
ongoing basis by reviewing policies and procedures
relating to that function and make necessary revisions to
initiate Quality improvement measures in a prioritized
manner.

3. To improve patient care guidelines relating to operative


and other procedure, in a collaboration effort.

4. To utilize a standard format for documenting and reporting


all Quality measures hospital-wide

5. To collect data on staff views regarding Quality


improvement activities

6. To develop a formal tool for prioritizing Quality


improvement activities

7. To strive to raise the benchmark in all aspects of service


delivery and meet the quality standard expected for the

HUMAN RESOURCE DEPARTMENT

Location: Basement

Objectives:
 Human resource management(HRM) is a means to
achieve efficiency and effectiveness. It serves other
functional areas to help them to attain efficiency in their
operations and attainment. Acquiring right man for the
right job at right time in right quantity, developing
through right kind of training, utilizing the selected
workforce, and maintaining the workforce are the
organizational objectives of HRM.

 HRM performs so many functions for other


departments. However, it must see that the
facilitation should not cost more than the benefit
rendered.

 HRM must see that the legal, ethical, and social


environmental issues are properly attended to.
Equal opportunity and equal pay for equal work are
the legal issues not to be violated.

FUNCTIONS:
 The success of recruiters and employment
specialists generally is measured by the number of
positions they fill and the time it takes to fill those
positions.
 One of the main functions of HR is to support
workplace safety training and maintain federally
mandated logs for workplace injury and fatality
reporting.

 Employee relations is the HR discipline concerned with


strengthening the employer-employee relationship
through measuring job satisfaction, employee
engagement and resolving workplace conflict.
MEDICAL RECORDS DEPARTMENT

Location: Basement

Work flow of MRD:

 First level of file auditing done in wards or floors


 Deficiencies are found

 Intimated to concerned Personnel

 Corrected & completed by concerned personnel

 Files sent to MRD within 48hours of discharge by the Floor


Manager

 Files are received in MRD by MRD executive

 Files are assembled according to discharge date

 Medical records are sorted according to normal discharge, MLC


& death

 File indexing are done

 Second level of deficiency check is done and again intimated to


concerned personnel

 Medical record not having ICD coding is done

 Normal discharge medical records are stored in the storage area,


death & MLC are stored separately under LOCK & KEY.

 Medical records are scanned by the concerned personnels (of


process infotech- an out sourced agency) & stored in DVD/CD/Hard
Disk

 Hard copy of the medical records are sent to

 crown – an out sourced agency.


OUT PATIENT DEPARTMENT

Location: 1st floor


PATIENT AND VISITORS

Visitor Information
It is regarded as an important part of the patient’s day.
Unless otherwise indicated, it is assumed that patients are
agreeable to visits from family and friends at the scheduled
visiting times. This guidance document reflects a balance
between the needs of patients, their visitors, hospital staff
and hospital procedures.
Purpose
 To manage the visiting arrangements so that a balance
between the needs of patients and visitors, and the needs of
hospital staff delivering treatment and care is achieved.
 To maintain a safe, secure and comfortable environment for
patients and staff.
 To enhance patient confidentiality.

Procedures and Guidelines:


Visiting in General Wards / Semi Deluxe/ Deluxe / Super
Deluxe / Suite
 Visiting hours are open

 The Visitor will carry the visiting card during each visit

 Recommended visiting time is 20 minutes.

 No more than two persons should visit a patient at one point


of time .

 Children under 12 years of age will be allowed only on


Wednesdays and Sundays.

 Children must be under the supervision of an adult at all


times while in the hospital.

 The Floor Manager can request that visitors leave, if the


number of visitors present or the length of the visit is
deemed excessive
 Additional restrictions will apply from time to time , for
example , during outbreaks of infectious diseases.

 Certain areas may implement stricter and more controlled


visiting policies to suit the needs of patients in that
particular ward or unit .
 Visiting hours may also be restricted for emergency medical
situations . All exceptions and restrictions are at the
discretion of the management.
 Relatives and friends are requested not to visit if they have
sore throats , flu , vomiting and /or diarrhea or other
infections , because of the risk of introducing infection to the
ward .

Visiting in the Critical Care Units such as


WICU /PICU / NICU / SCBU/Isolation
Visiting time is as follows:
In the morning: 9:00 AM – 9:30 A
In the evening: 4:00 PM - 5:00 PM

 As many patients are ill, visiting is restricted to immediate


family members for short periods of time only, with only one
person at a time.
 Visitors may be requested to leave the ward/unit during
procedures and thus be sensitive to the needs of the patients,
medical and nursing staff.

Patients Right to decline Visitors


 On receiving a request for restricted / no visitors from a
patient , next of kin , or nominated family member , the
nursing staff will document the request in the nursing notes
with the date and duration of such request .

 The nurse will inform the Floor Manager who will maintain a
record of such requests for the duration of the patient stay,
unless the request is subsequently amended.

 The Floor Manager will keep the Reception and Admission


informed of such request and advises the visitor that it is not
appropriate to visit and visitors are politely asked to comply
with this request.

TPA & CASHLESS FACILITIES

Location: Ground floor


Working process:
In case of emergency admission, patient /patient
relatives are supposed to produce a valid TPA ID
card or insurance policy copy.
The photocopy of the TPA ID Card or insurance
policy is retained by the admission desk while the
patient starts receiving necessary medical
treatment.
During admission the relative fulfills the required
formalities like filling up the necessary consent
forms.
The co-coordinator/doctors who is at the hospital
TPA desk fills up the pre authorization form and
forwards it to the concerned TPA by fax/ mail 24 to
48 hours of hospitalization of the patient.
The coordinator at the TPA desk does regular
follows-up with the concerned TPA and waits for
the initial approval letter.
The coordinator at the TPA desk answers to any
query/ additional information asked by the
concerned TPA.
After receiving the authorization/ denial letter or
any information from the concerned TPA, Co-
ordinator at the TPA desk informs the patients
relatives.
If the concerned TPA sends a denial letter before
the discharge of the patient, the patient/patient
relatives will have to settle the entire bill amount
by cash payment.
If the authorization letter is sent by the concerned
TPA the patient/ patient relatives have to pay the
charges of only the unapproved items.
At the time of discharge patient relatives will get
the photocopy of all reports. Original reports and
bill are sent to the concerned TPA.
In case of any Query the patient relatives may
contact the PRO at the front office.
