Please read the Terms and Conditions for using PERN2 service and the following statements carefully
before signing.
1. Any errors and/or omissions in form or incomplete form may cause delays in
processing or rejection of shifting.
2. Organization fully understands all relevant costs that may be charged when
subscribing to the PERN service.
3. Organization shall fully comply with all terms and conditions, Acceptable
Usage Policy (AUP) and Network Connectivity Policy (NCP), which will be
immediately applicable upon approval of service subscription.3
4. Organization agrees that PERN reserves the right to terminate this subscription,
without advance notice, in the event if organization breaches any of the above-
