AFFIDAVIT OF JUSTIFICATION/EXPLANATION

I, FILIPINAS CADAOAS-CADAOAS, of legal age, widow, American Citizen and a resident


of 32-B La Paz, Laoag City, Ilocos Norte, Philippines, after having been sworn to in accordance
with law, hereby depose and state:

1. That I am a former natural-born Philippine citizen, born on March 15, 1944 at Brgy. 32-B
La Paz, Laoag City, Ilocos Norte to Spouses Nazareno Cadaoas and Esmenia Sabado;

2. I lost my Filipino Citizenship when I was naturalized as a citizen of the United States of
America on January 25, 1978 ;

3. I intend to stay in the Philippines for good. I would also like to retain and reacquire my
Philippine Citizenship under Republic Act No. 9225;

4. As requirement I have executed my Oath of Allegiance to the Republic of the Philippines


on May at May 15, 2019, under Doc. 188, Book No: 38, Page No: XXI, Series of
2019before the Notary Public of Atty. Adel Cristina Z. Diño of Laoag City, Ilocos Norte,
Philippines;

5. That the reason why I did not take my Oath of Allegiance infront of the Bureau of
Immigration-Manila due to old age and I have difficulty in walking and currently
supported by a wheelchair. Hence, traveling by land or by air from Laoag City to Manila
would be difficult on my part and hazardous to my health; and

6. That I am executing this affidavit to attest to the truth of the foregoing and for whatever
legal purposes it may best serve.
