SUPPLEMENTAL AFFIDAVIT

I, MIA AMOR C. MAGAT, Filipino, of legal age, single and


with residence and postal address at No. 9, Lualhati Brgy.,
Baguio City, after having been duly sworn to in accordance with
law, hereby depose and state that:

1. I am the daughter of Lolita D. Cristobal;

2. My mother was born on August 31, 1962 at the Baguio


General Hospital, Baguio City from Spouses Alejandro
M. Cristobal and Erlinda C. Dalusong, who got married
sometime on December 12, 1962 at Tarlac;

3. Since birth, my mother had been using the name LOLITA


DALUSONG CRISTOBAL in all her scholastic records and
in all her public and private dealings. Never that she
used the name BABY GIRL DALUSONG CRISTOBAL as
appearing in her Certificate of Live Birth issued by
the Local Civil Registrar;

4. However, when I secured a copy of her Certificate of


Live Birth from the National Statistics Office, I
discovered that her name is BABY GIRL DALUSONG
CRISTOBAL instead of LOLITA DALUSONG CRISTOBAL as
appearing in her Birth Certificate issued by the Local
Civil Registrar. A copy of her Certificate of Live
Birth issued by the Local Civil Registrar of Baguio
City is hereto attached as Annex “A” to form part
hereof;

5. That her TRUE & CORRECT name is LOLITA DALUSONG


CRISTOBAL and not BABY GIRL DALUSONG CRISTOBAL,
which she never used in all of her private and public
dealings;

6. As such, I am executing this Affidavit to attest to


the truth of the foregoing facts and declarations and
to supplement the fact that my mother’s true and
correct name is LOLITA DALUSONG CRISTOBAL, for the
purpose of correcting her name appearing in her NSO
Birth Certificate and for all legal intents and
purposes it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 15 th


day May 2019 at Baguio City, Philippines.
