SWORN STATEMENT

I, MIRALYN J. PENASO, of legal age, married, Filipino with office


address at the Pototan Farmers Multi-Purpose Cooperative, Matias Yusay
Street, Pototan, Iloilo, after having been sworn to in accordance with law,
hereby depose and say:

I am the General Manager of the Pototan Farmers Multi-Purpose


Cooperative;

As a multi-purpose cooperative, we are issuing loose-leaf official


receipts and/or invoices to our members who are transacting with the
cooperative. The following official receipts and/or invoices shall be used, to
wit:

We hereby undertake to permanently bind the loose-leaf official


receipts at the end of the taxable year or upon termination of its validity.

We execute this sworn statement to attest the veracity and


truthfulness of the foregoing facts and circumstances.
