This document is an addendum to an existing contract between Kairosmed Diagnostic Center and Philippines Sanitary Fittings Inc. and Ryuma Ramen Inc. It modifies the original contract to include blood typing in annual physical examinations. Payment for services rendered to Ryuma Ramen Inc. will be collected from Philippines Sanitary Fittings Inc. No other terms of the original contract are changed. The addendum is signed and acknowledged by representatives of all parties.

ADDENDUM TO AN EXISTING CONTRACT

An agreement was made by and between KAIROSMED DIAGNOSTIC CENTER whose address is
#121 Don P. Campos Avenue, 2/F Agricultural Bank Building, Bgy. Zone IV, Dasmarinas City, Cavite
4114 and PHILIPPINES SANITARY FITTINGS, INC. and RYUMA RAMEN, INC. whose
addresses are as follows:

 PSF – FCIE, Langkaan, Dasmarinas City


 PSF – LTSP 3, San Rafael, Sto. Tomas Batangas
 Ryuma Restaurant 2nd Floor Paseo De Sta. Rosa 4 Barangay Don Jose Sta. Rosa City, Laguna

WHEREAS, the parties wish to modify the terms of the original stated contract hence blood
typing was included in the Annual Physical Examination.

NOW THEREFORE, in consideration of the mutual promises herein, the parties, intending to be
legally bound, hereby agree that the following constitutes additional terms and conditions of the
stated contract.

1. Transactions/billing shall be c/o to Philippines Sanitary Fittings, Inc.


2. Payment for Ryuma Ramen Inc. shall be collected from Philippines Sanitary Fittings, Inc.

No other terms or conditions of the above mentioned contract shall be negated or changed as
a result of this here stated addendum.
