Greetings of peace and prosperity!

I, the school head of Mahayag National High School formerly the San Miguel
National Comprehensive High School- Mahayag Annex would like to inform you that
our school MOOE is still under the management of the Surigao Del Sur Division.
During the third quarter period which is from July to September 2017, the MOOE is
downloaded last July 2017 and the liquidation is submitted on October 09, 2017
since the liquidation is done quarterly.

With this connection, the said division office is responsible on the remittance
of our taxes for the said quarter since the school is still under the Surigao del Sur
Division.

Attached herewith are the documents for further details. I am hoping for a
positive response to this matter. Thank you and more power!
