APPLICATION PROCEDURE GENERAL GUIDELINES
1. Request the Registrar’s Office of your school to accomplish the Grades Form. 1. Only applicants who are currently enrolled in Grade 12 may apply.
Make sure that the school principal or registrar signs. 2. The following applicants must secure clearance from the UST Office of the
Registrar before paying the application fee:

3. Proceed to any Metrobank or BPI branch to pay, over the counter, the non- Alternative Learning System Accreditation and Equivalency

2. Compute for the average of all interrelated subjects taken within the same
English Mathematics Science GWA school year.e.g. [Earth Science (85.00) + Science and Technology (87.00)]/2 =
Science
Grade 11 3. Record the average grade for English, Mathematics, and Science in the
appropriate boxes.
4. State the grades in standard quantitative equivalent (e.g. 85.00, 88.50).
5. General Weighted Average (GWA) for Grade 11 will include ALL subjects taken
by the student in Grade 11, not just in English, Mathematics, and Science.
6. For schools with a basic education program that does not follow the Department
of Education’s K-12 program but is recognized bt DepEd to be its ewquivalent,
