ACTIVITY 3: ONLINE SURVEY

A local restaurant distributes a “Customer Satisfaction Survey” to get feedback from


their customers. Read and study the survey questionnaire and create an online version
of it using an online survey tool.
Step 1: Sign up to a free online survey tool. You will need an email address to register.
Most online tools have almost the same command tabs used in building questionnaires.
For this activity, SurveyMonkey® will be used to help you design your online survey.

Step 2: Click on “Create Survey” to begin your project.

Step 3: Give your survey a title and choose the appropriate category.

Step 4: Add page title by clicking on “Edit”. Type in the “Page Title” box.

Step 5: Slide down the Builder tab to choose the question format. Note how each format
is like and how each one should be used appropriately.
Step 6: Add a “Rating” type question to the form. To come up with a horizontal rating
design, tick on the box “Make this a single rating scale (remove row choices).

Step 7: Add another “Rating” type question but this type you need to include both
horizontal and vertical rating scale.

Continue building the form by adding more questions using the appropriate question
types.

Step 8: After designing the form, you are ready to share it to your target respondents.
Respondents are those people who will answer your survey. To do that, click the “Next”
button and you will be directed to the “Web Link” page.

Step 9: Send the link to your classmates and have them answer the survey form. Wait
for them to respond to the survey. One of the advantages of using online survey tools is
the ease of generating results. Find out about the result of the survey by clicking on the
“Analyze Results” tab. The result is presented through graphs which make it easy to
understand.

Step 10: Review and analyze the results. Answer the following questions:

a. Which options per question earned the highest points and lowest points?
b. How will the results help the restaurant’s management in improving their service?
c. As a young entrepreneur, how will you use online survey in achieving your goals?

CHALLENGE:

To be a successful entrepreneur, you must take time to evaluate the product or services you
offer. Take time to listen to the feedback of your customers regarding the quality, packaging and
even the advertising strategies you employ to promote your product or services.

To do this, you are going to conduct a survey to know the customer’s feedback. Consider the
following in planning your questionnaire:

Content:

1. How was the quality of the product/services?


2. Did the packaging/presentation able to catch the interest of the customers?
3. Was the price of product reasonable/competitive?
4. Is the advertising strategy using online tools effective?

Design:

1. How many questions will you include in the survey to be able to get all information that
you need?
2. What type and form of questions will you include?

After planning the design and content of your online survey, create one and share the link to
your schoolmates.

Show to the class the process of creating this online survey.
