GENERAL INSTRUCTIONS
1) Submit two (2) copies each of the duly accomplished Application Form and Undertaking. Incompletely filled forms
cannot be processed. The Undertaking should be notarized.
2) Pay the P300 application fee when the application is submitted. Use postal money order or checks payable to the
“Legal Education Board” for applications filed through mail or courier.
3) Submit with the application a self-addressed and stamped envelope or prepaid pouch if you want the
“Conditional Admission/Enrollment Permit” sent to you by mail.

That, I am intending to enroll in the basic law course at the beginning of the First
Semester of the Academic Year 2019-2020 in the law school of (supply the name of the
university/college in the blank space ahead) _

That, if conditionally admitted by a law school and enrolled in the basic law course by
virtue of a permit issued in accordance with LEB Memorandum Order No. 11, Series of 2017
pursuant to this application, I expressly agree that my said conditional admission and
enrollment shall be subject to the conditions indicated in the UNDERTAKING attached hereto
as Annex “A”:
