SUBJECT : Spot Report re- Found Dead Body

DATE : October 3, 2019

1. Reference: Standard Operating Procedure (SOP).

2. This pertains to above subject that transpired at block 25 F Lot 31 Southville 9, Brgy.
Pinugay, Baras, Rizal wherein the victim identified as Lerma Beronio y Alarcon, 21 years old,
born on November 21, 1997, POB: Patnanungan, Quezon, female, with live-in partner, jobless,
a resident of block 25 F Lot 31 Southville 9, Brgy. Pinugay, Baras, Rizal. Witness one (1)
Chabelita Moneda y Padiragao, 20 years old, born on August 9, 1999, POB: Daraga, Albay,
with live-in partner, jobless, female, high school level, a resident of block 25 lot 28 Southville 9,
Brgy. Pinugay, Baras, Rizal. Suspect was unidentified.

3. On or about 9:00 AM of this date, a telephone call received coming from a concern
citizen that have an alleged dead body inside the given address. Personnel of this Office
proceeded to the area to verify the veracity of said report and upon arrival thereat the following
gathered to wit. Investigation disclosed that on or about 7:00 AM of October 3, 2019 while the
abovementioned witness noticed that some of jalousies of the window of said victim was
detached, thus she peek inside to look of her friend (victim) to verify and when she came in
suddenly saw the body of the victim lying on the bed without lower garment and the face
covered by pillow and believed it was dead.

4. Further, this Office seeks the assistance of Rizal Provincial SOCO Team for technical
process of possible evidence if there was a foul play on the death of the victim. The team led by
PMAJ JOAN ROSALES arrived at about 11:10 AM and started processing the crime
scene that ended at about 12:10 PM of same date. This office continuously conducting
follow-up investigation.
