On a day at Mitra Delima Hospital is a patients woman name is Resi treate in Melati
room with interference breathing, th nurse Izza and nurse Dina will do action check vital sign
(Nurse Izza and Nurse Dina doing visiting the patient)
Dina: Good afternoon miss
Resi : Good afternoon too suster
Dina: What is your name ? ( looking at the bracelet )
Resi : Ms. Resi
Dina: Oh yes really means with Ms.Resi
Izza : Introduce ya I nurse izza and this nurse dina , here we will be do examination
of signs vital, purpose to determine whether the signs of vital limits normal. Do
you ready ?
Resi : Yes ready suster
Dina: How are you now ? Does it still feel breathless ?
Resi : Yes suster, still feels a little breathless
Imel : Yes suster, last night my child can not sleeped because of its breathless
Dina: Oh yes mom , I will measure blood pressure and temperature first , mom
Imel : Yes please suster
(Dina doing action measure the pressure of blood and temperature are simultaneously )
Imel : How many suster ?
Dina: 100/80, still within the limits of normal mom , the temperature is also within the
limits normal 36.4 C mom. For the measurement
of pulse and respirasinya be measured of nurses izza yes mom
Izza : Excuse me mom, I will measure the pulse and respiration of the mother child
Imel : Please sus
Izza : Pulse 86x / minutes while the respiration of its 28x / minutes mom. Because the child 's
mother breathless.
Imel : Then, How manner resolve breathless relapses when in home suster?
Izza : I will teaching the techniques of relaxation breathing yes mom,
so that could in practice at home
Imel : Yes sus
Izza : With the position of the semi fowler then pull the breath through your nose , hold until
4 seconds and exhale it slowly . Try mom and ms.Resi practice
(Imel and children Ms.Resi do practice the techniques of relaxation breathing )
Izza : It is one of the actions that can be done at home mom when at times claustrophobic child 's
mother relapse . Any question again mom ?
Imel : Nothing suster. Thank ya sus
Izza : Yes mom you’re welcome , if nothing question we are excuse me yes mom
Imel : Oh yes suster
Pada suatu hari di rumah sakit mitra delima ada seorang pasien perempuan bernama Resi dirawat
di ruang melati dengan gangguan pernafasan, perawat Izza dan Dina akan melakukan tindakan
pemeriksaan tanda-tanda vital. (perawat Izza dan Dina menghampiri pasien)

Dina : selamat siang mbak

Resi : selamat siang juga mbak

Dina : sebelumnya nama kamu siapa? (melihat gelang)

Resi : Resi mbak

Dina : oh iya benar berarti dengan mbak resi

Izza : perkenalkan mbak saya perawat izza dan ini perawat dina, disini kami akan melakukan
pemeriksaan tanda-tanda vital, tujuannya untuk mengetahui apakah tanda-tanda vital dalam batas
normal. Apakah mbak bersedia?

Resi : iya bersedia

Dina : bagaimana keadaan mbak sekarang? Apakah masih terasa sesak?

Resi : iya mbak masih sedikit terasa sesak

Imel : iya mbak tadi malam anak saya tidak bisa tidur karena sesaknya

Dina : oh iya buk, saya ukur tekanan darah dan suhunya dulu ya bu

Imel : iya sus silahkan

(Dina melakukan tindakan pengukuran tekanan darah dan suhu secara simultan)

Imel : berapa sus?

Dina : 100/80, masih dalam batas normal bu, suhunya juga dalam batas normal bu 36,4 C. Untuk
pengukuran nadi dan respirasinya akan diukur oleh perawat izza ya bu

Izza : permisi bu saya akan mengukur nadi dan respirasi anak ibu

Imel : silahkan sus

Izza : nadinya 86x/mnt sedangkan respirasi nya 28x/menit bu karena anak ibu sesak

Imel : lalu bagaimana sus saat sesaknya kambuh saat di rumah ?

Izza : saya ajarkan teknik relaksasi nafas dalam ya bu supaya bisa di praktikkan di rumah

Imel : iya sus


Izza : dengan posisi semi fowler lalu tarik nafas melalui hidung, tahan selama 4 detik dan
hembuskan secara perlahan. Coba ibu dan mbak Resi praktikkan

(imel dan anak resi mempraktikkan teknik relaksasi nafas dalam)

Izza : itu salah satu tindakan yang bisa dilakukan di rumah bu apabila sewaktu-waktu sesak anak
ibu kambuh. Apa ada yang ditanyakan lagi bu?

Imel : tidak sus terimakasih yaa sus

Izza : iya bu sama-sama, kalau tidak ada yang ditanyakan kami permisi dulu ya bu

Imel : oh iya sus
