Thank you for seeing Ms Day, a 20-year-old university student with part-time job, whose features are Comment [teacher1]: a
consistent with some neurological damage. Your further investigations and appropriate management is
requested.

Ms Day has a past medical history of various viral infections and endocrinal issues. She is a smoker, drinks
in moderation and has no family history of headaches or migraines. She is on an OCP.

Initially, on 06.12.17, she presented with steady, non-throbbing unilateral headache and visual
disturbances. She reported 2 episodes in past 2 weeks with aura and that symptoms lasted for 1-2 days. Comment [teacher2]: the
However, her neurological examination was unremarkable.

On her next follow-ups, there was no improvement in her symptoms and she complained of more frequent
attacks. Additionally, her mother claimed that the patient seems to appear depressed and her medications Comment [teacher3]: seemed
(eleptriptan and ibuprofen) along with rest and ice weren't effective anymore. Ms Day is currently on
amitriptyline 25 mg b.d and OCP.

Her recent visit revealed that she had parasthesia in the fourth and fifth fingers of her left hand. She also
revealed that she sustained a whiplash injury last year, which I believe is the cause of her symptoms. No
medical attention was sought at that time.
