This report follows a health and safety inspection of xxxxx conducted on xxxxx . It is an aided private
institution run under the financial assistance of central and state government .It conduct at present
three year diploma courses in , civil, mechanical, electrical ,and electronics. In this institution there
are about 750 students and 150 faculty. There is a common general workshop with different branches
such as , Lathe, Welding, carpentry .Apart from there is an Administrative office, welfare facilities,
rest rooms, lecture hall, and canteen. In lathe workshop consist of a late machine and milling machine
which include the operations like, Centering, straight turning, facing, chamfering, knurling, boring, etc.
in welding workshop consist of an arrangement of Oxygen and acetylene gas cylinders for different
types of welding.in carpentry workshop included the operation like , sawing, planning, chiselling,
moulding etc. The college management also provided a canteen for staff as well as student. The
administration office deals with student registration ,enquiries, funds etc. Inspection included all the
above mentioned department in the college there is no exclusion criteria. The normal working hours
defined by the college management is 9am to 4pm, from Monday to Friday.
Executive Summary
The xxxx college is the one of the three polytechnic college in Kerala started in 1958.Since 1958 no
major accident has been recorded only minor events has occurred during this period .Even though
this is appreciable proper implementation with regards to safety policy
And working protocol should be maintained regularly to avoid major hazard in near future. The
management also conduct frequent safety check not only to the staff and student, but also existing
equipment and materials in the workplace especially prior to use. Proper safety measures should
be implemented and has to be inspected regularly. The major control measures should be
implemented as early as possible .This will prevent the enforcement action and also protect the
reputation of the college. During the inspection there have been some important findings that have
raised concern regarding the health and safety of students ,staffs , and others involved in the daily
activities at the college premises .These findings breach the health and safety legislation in various
ways.
There is a fall hazard at main entrance of the college stairway which is broken people ascending or
descending the stairway may fall down by losing balance and get fractures, bruises, abrasions, sprain
and other musculoskeletal problems or even cause death by their head striking against the edge of
This is a breach of C155 occupational safety and health convention 1981 [no.155] which
state that “The provision of design, siting, structural, features, installation, maintenance,
repair, and alteration of workplace and means of access there to and egress there from are
safe”
If the enforcement agencies identified this issue the college will have to pay fines and face
charging in the court.
In order to control this hazard and prevent further accidents immediately we have to barricade
the area and place the sign board of warning of damaged steps. We also need to implement a
guardrail system on the both side of the stairways, when repairing the stairway it is advisable
to provide slip resistance floor or recoating decks or adding anti slip strips to stairway and on
rainy days take additional control measures showing the danger of slipping surface. During
the repair of damaged stairway a ramp should be constructed for the disabled .Additionally 2
or 3 wheel chairs with adequate braking system have to be procured for the disabled.
There is a lathe machine and vertical milling machine in general workshop which are operated
by the workshop superintendent and trainee students. The machines have not been serviced
in the recent past nor is there any maintenance schedule defined for these machines. The
machines are not maintained as per the manufacturer recommendation this has resulted in
wear and tear to the parts of the machines and the wear and tear is causing the generation of
heat due to friction of the moving parts. Oil is being leaking from the hydraulic arm of the
vertical milling machine and causing slipping as well as environmental pollution there is a
major chance of fire due to this hazard.
The rotating parts of the machines have no guards to prevent contact with the moving parts
and students as well as tutors use the machines while carrying loose clothing, jewellery, and
some of them also have long hair which may get entangled with moving parts and thus cause
the injury. There is also a hazard of flying particles from the work-piece when the machine
being operated.
This is a breach of legislation as per ILO convention C167 Article 17 which state that

far as possible ergonomic principles.

3. adequate instruction for safe use shall be provided where appropriate by the manufacturer
or the employer in a form understood by the users.
We are in breach of the law the enforcement agencies identified this issue
They may issued prohibition notice for that particular unsafe machine, also issued penalty ,
improvement notice, and even imprisonment or prosecute the college authority. If an accident
occurs the students or the tutors may go to the court and will be able to prove that the college
authority were negligent with regard to their Health and Safety responsibility and this will lead
to pay huge amount to the injured person as well as other hidden costs.
It recommended that the use of the machine should be prohibited until it’s maintained and
serviced properly as per the manufacturer standard. There is a requirement to plant and
For protecting from the rotating parts and the projectiles there is a guard have to be provided
with provision for easy maintenance.
There should be a spill management plan produced and the cleaning staff should be trained
in this procedures to clearance the spillage immediately were ever it is occur. Additionally
there should be administrative procedures should be placed to prevent the machine from
being operated while wearing jewellery , or loose clothing and if the person is having long
hair this can be in the form of signages and return notifications and memos.
During the inspection at the kitchen area it was noted that the kitchen waste was not
segregated and was dumped at an un designated waste disposal area near the main cooking
section. The waste consist of non-degradable waste such as remains of chicken, meet, fish,
as well as remaining and stale portion of vegetables there is also oils and the insoluble waste.
It was noted that rodents and insects such as flies , rats, were feeding on these waste, this
can cause to spread of diseases due to the bacteria being transmitted by the insect and
rodents at the rest room area.
The toilets were observed to be filthy and emitting bad order and also there is no hand-wash
facilities in rest rooms. This may result in the users being contracted with diseases such as
Hepatitis A, B, etc. At the administration office the drinking water cooler has placed and only
one glass has been provided for drinking purpose this glass is used by all the students and
staff for drinking water. This can cause salivary cross contamination and result in spread of
diseases. If any diseases occur to even one person due to any of these issues it will spread to
the others with in a short period of time that will cause a major emergency.
This is a breach of legislation as per ILO convention C167 Article 28 which state that “Where
a worker is liable to exposed to any chemical, physical, or biological, hazard such an extent
as is liable to be dangerous health appropriate preventive measures shall be taken against
such exposure”
It recommended to provide a designated waste disposal away from the kitchen area and
proper segregated waste can be put in skips or containers with lids. There has to be a system
in place to contract the waste disposal to a third party or organization on specialized in these
field constant monitoring also put in placed to ensure the waste is removed art regular
intervals.
For the issue with the toilets a cleaning staff must be recruited with adequate personal
protective equipment and cleaning chemicals should be purchased and regular cleaning
schedule to be implemented and it’s effectiveness should be checked frequently. A hand
wash facility should be provided with hand drier facility and sanitising gel or liquid.
Disposable glasses should be provided at the dispenser area so the usage of single glass by
multiple people is avoided.
During the inspection it was noted that some fire hazard were present at several areas within
the college premises. Oxygen and acetylene gas cylinders are kept outside the welding
workshop on an inclined surface in an upright position. This may cause the cylinders may
tumble down and lead to gas leakage fire and/or explosion resulting in injury to workshop
employees such as burns and also lead o huge property damage.
Another observation was that fire extinguisher were not available at any location all over the
place for fighting purpose if an accident occur, this may lead to a situation where the fire can’t
be controlled and lot of people get injured and lot of property will get damaged there will be
danger to workshop and to the structural parts of the building resulting for huge amount of
costs for the repairs and compensation to the injured people.
The cost associated with the repair can’t be reclaimed because there is no insurance
coverage. The reputation of the college will go down and there will be legal procedures like
penalty, imprisonment etc.
This is a serious breach of legislation as per ILO convention C167 Article 29 which state that
“ “ The employer shall take all appropriate measures , to avoid the risk of fire, combat quickly and
Efficiently any outbreak of fire, Sufficient and suitable storage shall be provided for flammable liquids,
Solids and gases.”
It is recommended that the Oxygen and acetylene gas cylinders removed immediately from
the current inclined surface location to a temporary storage and also secured by using
chains. A permanent designated area should be constructed away from the building which
has proper ventilation shade flat floor surface and lockable fences gate , chain should be
provided to protect the cylinder in upright position. There should be separate facility for empty
as well as fully cylinders and further separation for oxidisers and the flammable gas , hot work
prohibited and entry restricted to authorised personnel. Smoking should be prohibited in the
campus or a separate smoking booth constructed and provide with adequate fire
extinguishers in place.
For the issues with the lubricants and oils and chemicals were kept on work bench is not
allowed, construct designated chemical storage area for these chemicals with all appropriate
facility with bund.
Electrical hazard such as open electrical panel box , broken wires which have been joint and
covered with insulation tape and also having scorch marks were observed at general
workshop area. This may cause electrocution and electrical fires leading to injury and
property damage
This is a serious breach of legislation as per ILO Convention C167 Article 26 which state
that “ All the electrical equipment and installation should be constructed , installed and
maintained by a competent person and so used as to guard against danger.

Which may result in fines and prosecution leading to lose of approval of the college and also
result in costs due to the property damage and insurance compensations.
It recommended that only authorised people should work on electrical system and the
procedures being implemented to ensure that lock out/tag out is implemented and all the
electrical equipment are return to normal working condition after work. A sign board is placed
on the panel box stating that should be operated by authorized people only and it should be
closed after work .The damaged electrical cables removed immediately and new ones which
are inspected and tagged should be used. Where there is a requirement for the extending
Conclusions
The workshop were find to be clean tidy there is no issue with housekeeping also the college
management provide a good ventilation system all over the college buildings. The students were
enjoyed to come workshop and do their own work practice under the observation of workshop
superintendent. Even though there were a number of issues which need to be rectify immediately to
avoid serious accidents and compensation claims following the resultant injuries. The most significant
hazards identified in this report related to, Falls from height, Machinery Hazards due to poor
maintenance, Fire hazard, Biological hazard due to un hygienic conditions, and Electrical hazard.
The ascending and descending of people on broken stairway without guard rails must be prohibited
immediately until it maintained properly. Work on the poorly maintained and unguarded machines
such as late and milling machines were must stopped immediately until the machines are both
properly maintained and serviced as per manufacturer recommendation and machines are guarded
properly. Un hygienic condition of toilet and kitchen area were secure immediately as per the
recommendation this include purchasing of chemicals and recruiting a cleaning staff. Work on broken
wired electrical equipment must be prohibited and replace the broken wires with new one and
purchasing of industrial socket rather than insulated joint wire. There is limited availability of fire
extinguishers all over the college premises for fire fighting if an accident occur this may lead to a
situation were the fire can’t controlled in order to control the situation the college management must
buy adequate numbers of fire extinguishers.
Also the report has identified a number of breaches of health and safety law which left could lead to
action by enforcement authority. The majority of recommendations made in this report are low in
terms of money, time, and effort. The implementation of the recommendation made in this report will
help to reduce the potential for an accident as a result of which costs associated with accident and ill
health will decrease.
