Affidavit of Undertaking
I, ______________, of legal age, single, Filipino, and presently residing in
Sitio Ubos, Poblacion, Cordova, Cebu, after having been duly sworn in
accordance with law, hereby depose and say:

1) That I am the owner of a RUSI Swish Mono ZX12 motorcycle with


Certificate of Registration (C.R.) No._____________, Official Receipt
(O.R.) No. 1374561252 and Plate No. ___________;

2) That likewise I am the accused in People v. _____________ docketed


as ______________before the Regional Trial Court-Branch 88 of
Mandaue City for violation of Section 11, R.A. 9165. I was riding my
abovementioned motorcycle when apprehended for the offense
charged. Consequently, the said motor vehicle has been impounded
and is currently under the custody of the Bogo City Police Station;

3) That I have moved for the release of my motorcycle before the


Honorable Court;

4) That should the motorcycle be released to my possession, I hereby


undertake to immediately surrender it upon order of the Honorable
Court for proper administration of justice.
