We, Virgilio G. Fronda , Filipino, of legal age, married with postal address at Angio, San Fabian,
Pangasinan; and Gina M. Fronda, of legal age, married with postal address at Angio, San Fabian,
Pangasinan, after being sworn to in accordance with law, depose and state:

1. That we have known GLADYS ROLLODA GAYETA since she was seven (7) years old as she is
the daughter our neighbor Mr. Ricardo D. Gayeta and Mrs. Beatriz R. Gayeta;

2. That GLADYS RULLODA GAYETA and GLADYS ROLLODA GAYETA is one and the same person;

3. That what we have stated above is true and correct according to the best of our personal
knowledge and belief;
