JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS


Re: ONE AND THE SAME PERSON

WE, VICTORINA T. REMOTO and VICTORIANA E. MELON, both of legal ages,


Filipino citizens, married, respectively and both residents of Sitio Bangcolutan, Barangay
Mayabon, Zamboanguita, Negros Oriental, after having been sworn in accordance with the law,
hereby depose and state that:

1. We personally know ANTONIETA EVANGELIO ELMA, who is our longtime


neighbor and acquaintance at Sitio Bangcolutan, and the fact of her birth is registered at
the Local Civil Registrar of Zamboanguita;

2. We know for a fact that the person by the name of “ANTONITA EVANGELIO” as
appearing in her Certificate of Live Birth procured from the Philippine Statistics
Authority (PSA) and the name “ANTONIETA EVANGELIO” as appearing in her valid
identification card, baptismal certificate, and legal transactions, REFER TO ONE AND
THE SAME PERSON and that her true and correct name is ANTONIETA
EVANGELIO ELMA;

3. We are not in any way related to any of the foregoing person within the fourth civil
degree either by consanguinity or affinity, nor do we have any pecuniary interests in this
matter whatsoever;

4. To remove confusion as to the two names, we hereby declare that the name
“ANTONITA” and “ANTONIETA” refer to one and the same person whose
registered name is ANTONIETA EVANGELIO ELMA;

5. We are executing this affidavit to attest to the truthfulness of the foregoing statements and
to establish the real identity of ANTONIETA EVANGELIO ELMA and for all legal
intents and purposes it may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 13th day of June, 2019 at
Dumaguete City, Negros Oriental, Philippines.
