Findings:
Folow-up examination to prior study shows interval progression of the intra and/or
extrapancreatic complex collection
involving the pancreatic head/proximal body. This acute necrotic collection extends
to the peripancreatic regions and
porta hepatis. It demonstrates homogeneous borders without a definite wal and
contains non-liquefied solid
appearing components. This has an aggregate measurement of about 10.0 x 13.9 x 14,4
cm, APxTxCC (previously
remeasured dimensions of 8.5 x 11.9 14.3 cm). Mass effect to the adjacent
structures are not significantly changed
The rest of the pancreas remains compressed. Pancreatic duct is not dilated.
Stomach is collapsed with an enteric tube in place. Some of the small intestines
remain distended and fluid. Bowel
gas pattern is nonobstructive. Minimal bilateral paracolic and pelvic fluid are
present. No evidence of
pneumoperitoneum.
The liver is not enlarged with smooth marginal contour. Parenchyma is homogenous.
No enhancing mass lesion
seen. Both intrahepatic ducts remain prominent, likely due to mass effect of the
aforementioned abnormal collection
Portal vein is patent
Spleen and both adrenal glands are normal in size and density. No focal mass
lesions detected.
The kidneys are normal in size and contour. No radiopaque lithiasis is noted,
bilaterally. There is prompt and
symmetric nephrogram enhancement in both sides. No focal enhancing mass is
demonstrated. The pelvocalyceal
structures and both ureters are not dilated
The aorta and iliac vessels are normal in calibre with adequate opacification
