APPLICATION FORM (Cadetship Program)


PROCEDURE:
1. Only true and correct entries on this form are accepted; any false information will be ground for disqualification.
2. Please complete the online application form in www.ntcm.com.ph or send them through snail mail @ NTCM Building Gate 2 Tesda Complex,
East Service Road Taguig City on or before December 1.
3. EXAMS:
Ability Profile Test , Professional learning indicator and Predictive Index
Written exam covers:
Math, English, Science and Inductive Reasoning
Wait for the call / text NTC- Manila for the “RESULTS”
4. SCREENING:
Successful examinees of the Pre - Qualifying exams will have to pass the following:
 Written/Computer Examinations
 Oral Interviews
 Medical Examination accordance to PEME, NMA, MLC, and STCW
5. Enrollment to NSA affiliate school. A successful qualifier shall be enrolled to any of the affiliate School of NSA (Davao, Bacolod
and Cebu)

QUALIFICATION REQUIREMENTS:
Applicants should/must be:
 16-23 years old on or before 30 April.
 High school graduate of School Year 2015 and below
 Existing college students and tertiary level graduates
 Graduates of Senior High School (K-12)
 Pass qualifying examination (written and computer exam)
 Physically and Mentally fit
 Natural Born Filipino Citizen
 Single
 No derogatory records or was convicted/accused of any crime of moral turpitude

Grounds for Disqualification:


 Defective visual acuity
 Defective Visual Perception (color blindness)
 Physical Deformities, Congenital defects
 Perforated ear drum, chronic sinusitis, goiter, bleeding tendencies, hemorrhoids, hernia (if not treated), and extensive skin disorders.
 History of heart disease, hypertension, asthma, active PTB, kidney, and liver diseases, epileptic seizure disorders, major operations,
communicable diseases (such as AIDS, Hepatitis, VD, etc.)
 Visible tattoo
 History of pregnancy, presence of breast mass and painful menstruation for females.

Bachelor of Science in Marine Transportation (BSMT) – Deck Officer
