PETITION

The undersigned, Alan Christopher S. Oliva, hereby applies to have the land hereinafter
described brought under the operation of the Property Registration Decree, and to have title thereto
registered and confirmed:

AND DECLARE…..

1. That the applicant is the owner of the land (by virtue of possession in accordance with
Section 14 (2) of P.D. 1529), together with the building and improvements thereon. The said land,
consisting of one (232 square meters) parcel, is situated, bounded and more particularly described,
as shown on the plan and technical descriptions attached hereto and made a part hereof, as follows:

“A parcel of land (Blk. 14 Lot 14 of the Subd. Plan PSD-04-002326, being a portion
of Lot 5-A, described on the plan (LRC) PSD-217590, LRC Rec. #11503), situated
in the Bo. Of Tungkong Manga, City of Malolos, Province of Bulacan, Island of
Luzon, bounded on the northwest, along line 1-2 By Lot 12, Blk. 14 on the
northeast, along line 2-3 by St. Lot 1-13 on the Southeast, along line 3-4 by Lot 16,
Blk. 14; on the southwest, along line 4-5 by lot 15, Blk. 14 and along line 5-1 by
Lot 13, Blk. 14, all of the Subd. Plan. Beginning at a point marked “1” on plan
being S. 74, deg. 17’W., 4.33 M. to the point of beginning; containing an area of
two hundred thirty two (232) sq. meters…”

2. That said land at the last assessment for taxation was assessed at Php. 400,000.00,
Philippine currency and the buildings and other improvements at Php. 500,000.00, Philippine
currency.

3. That to the best of my knowledge and belief, there is no mortgage or encumbrance of any
kind whatsoever affecting said land, nor any other person having any interest therein, legal or
equitable, or in possession other than the undersigned.

4. That the applicant has acquired said land in the following manner: that the undersigned has
been in open, continuous, exclusive and notorious possession and occupation of the land; that the
land the applicant occupies is declared as patrimonial property since January 5, 1966; and that the
undersigned and his predecessors in interest have been in the possession of the land since January
16, 1950.
5. That the said land is occupied by the following persons: the undersigned and his immediate
family.

6. That the names in full and addresses, as far as known to the undersigned, of the owners of
all adjoining properties, of the persons mentioned in paragraphs 3 and 5, and of the persons shown
on the plan are as follows:

a. Ms. Bing Bernardo, Blk 14, Lot 12, R. Salas St., Ph. 2B, Ciudad Real, Malolos
City, Bulacan

b. Ronnie Mangulabnan, Blk 14, Lot 13, Padre Faura St., Ph. 2B, Ciudad Real,
Malolos City, Bulacan

c. Ms. Joy Vega, Blk. 14, Lot 15, Padre Faura St., Ph. 2B, Ciudad Real, Malolos
City, Bulacan

c. Mr. Jonathan Danao, Blk. 14, Lot 16, R. Salas St., Ph. 2B, Ciudad Real, Malolos
City, Bulacan

7. That the applicant is single.

8. That the applicant’s full name, age, citizenship, residence, and postal address are as
follows: Alan Christopher S. Oliva, 23 years old, Filipino, residing at Blk. 14, Lot 14, R. Salas St.,
Ph. 2B, Ciudad Real, Malolos City, Bulacan 3023.

9. That the applied land is not bounded by a public or a private road.

10. That the following documents are attached hereto and made a part hereof:

a. Original Plan as Annex “A”, hereof;

b. Technical description as Annex “B”, hereof;

c. Tax Declaration, as Annex “C’, hereof;

d. Tax Clearance, as Annex “D”, hereof; and

e. Land Status, as Annex “E”, hereof.
