Re: Motion/Request for LIFTING THE SUSPENSION OF DRIVER’S


LICENSE

Sir:
This is in reference to Case No. __________________________ filed
against me by your office for the Suspension of my Driver’s License based on
the Notice of Suspension of Driver’s License with No. ___________________
on the ground of inappropriate Driver’s License Restriction Code. The Notice
is dated July 15, 20___ as hereto attached in this request.

I am hereby requesting that the Suspension be lifted for the following


reasons or grounds:

1. The Fine imposed therein has already been paid by me as


evidenced by Official Receipt No. _______________ dated August
_____ 2015, a photocopy of which is attached in this request;

2. For humanitarian reasons that Driving for a fee is my one and only
source of income in order to support my family; and

3. My Driver’s License is still valid to this date only that it has certain
restrictions such as (1) and (2).

In view of the foregoing valid reasons, I am most respectfully


requesting that the Order of Suspension of my Driver’s License be LIFTED.
