AFFIDAVIT OF DISCREPANCY
I, Lowell Rey G. Saliente, of legal age, Filipino citizen, single, a
resident of Purok Azucena, Amats St. Brgy. Taft, Surigao City, after
having been duly sworn in accordance with the law, do hereby state:

1. That, I am the legitimate eldest son of the late Renato


Pendejito, then a member of the Philippine National Police;

2. That, the record appearing on the Death Certificate of my


father compared to the data obtained by Tagum Cooperative revealed
that his middle name, was spelled as PENDIJITO instead of
PENDIJETO;

3. That, the full name of my grandmother as appearing on the


Death Certificate of my father was indicated as GLECERIA SACRO
PENDIJITO in contrast to the spelling found in my father’s Marriage
Contract, particularly on the portion indicating mother of the groom
which was indicated as GLECERIA PENDEJITO;

4. That, RENATO PENDIJITO SALIENTE and RENATO


PENDIJETO SALIENTE are one and the same person to the best of my
knowledge;

5. That, GLECERIA SACRO PENDIJITO and GLECERIA


PENDEJITO are one and the same person to the best of my knowledge;

6. That, since I came of reason, there’s no other person whose


been using aforementioned middle names in either of the foregoing
spelling;

7. That, since Elementary years, I’ve been referring my father


as RENATO PENDEJITO SALIENTE as appearing on my Birth
Certificate;

8. That, it was just recently that I discovered the correct


spelling of his middle name as appearing in his Birth Certificate; and

9. That, I attest to the truthfulness of the foregoing


declarations.

4. That, LOLITA GARCIA and LALITA GARCIA are one and


the same person to the best of my knowledge;

5. That, differences in spelling of the name of our mother in our


respective Birth Certificates may have been only clerical error;

6. That, LOLITA GARCIA and LALITA GARCIA are one and


the same person to the best of my knowledge;

7. That, since I came of reason, there’s no other person whose


been using aforementioned names in either foregoing spelling;

8. That, I attest to the truthfulness of the foregoing


declarations.
