This joint affidavit was made by Jeriel T. Mercado and Remie Joy L. Huston to attest to the late registration of birth of Adriel Bernice Mercado Barliso. They state that Adriel was born on March 24, 2019 to unmarried parents Lcee Joy Tupas Mercado and Rolando Rulida Barliso, attended by a midwife. However, due to negligence, her birth was not immediately registered with the Office of the Civil Registrar in Zamboanga City. They are close family friends and neighbors of the parents and are providing this affidavit to support the application for late registration of Adriel's birth.

JOINT AFFIDAVIT
(Re: Late/Delayed Registration of Birth)

We, JERIEL T. MERCADO and REMIE JOY L. HUSTON, both of legal age,
Filipino citizens and with residence and postal address at Culianan, Zamboanga City and
Pasonanca, Zamboanga City respectively, after having been duly sworn and oath in
accordance with law hereby voluntarily depose and say:

1. That we know the facts and circumstances surrounding the birth of the child
named ADRIEL BERNICE MERCADO BARLISO who was born on 24th of
March 2019 to her biological parents LCEE JOY TUPAS MERCADO and
ROLANDO RULIDA BARLISO;

2. That the said child was born outside of wedlock and the birth of said child was
attended by a midwife;

3. That due to the negligence and/or inadvertence on the part of his parents and
the prevailing conditions then, his birth was not reported and registered with
the Office of the Civil Registrar of Zamboanga City immediately thereafter;

4. That we have first-hand knowledge of the foregoing facts because we together


with our respective families, being their acquaintances, close family friends,
and neighbors;

5. That we are executing this Joint affidavit to attest to the truth of all the
foregoing facts, in connection with the application for the late/delayed
registration of birth of the said ADRIEL BERNICE MERCADO BARLISO with
the office of the Civil Registrar of Zamboanga City and for whatever legal
purpose this may serve;

IN TRUTH WHEREOF, we have hereunto set our hands, this December 3, 2019
at Zamboanga City, Philippines.
