SDSC Employment Contract
This AGREEMENT, entered into this _____ day of _____________, ________, between ST. DOMINIC SAVIO COLLEGE
represented by its President DR. NESTOR V. DELA CRUZ ("School/College"), and ________________________________
("Employee"), Whose address is _____________________________________________________________________________________.
WITNESSETH THAT:
WHEREAS, the parties hereto desire to enter into this Agreement to define and set forth the terms and conditions of the
employment of the Employee by the Company;
NOW, THEREFORE, in consideration of the mutual covenants and agreements set forth below, it is hereby covenanted
and agreed by the Company and the Employee as follows:
Within the period stated above, you will be evaluated based on your performance.
You are expected to report daily from Monday to Saturday which shall commence at 6:30am up to 5:00pm or
as determined by your shift to be approved by the President. As part of the security force, you may be assigned as
required to do works on Sundays and/or night shift/s as part of the rotation schedule. However, should you be assigned
as member of any Committee/Team of the school, you are expected to be on call or work beyond regular reporting days
and hours to finish an assignment or task given.
You should be present and fully participate in all institutional activities, as well as in departmental activities
and, in all meetings and gatherings scheduled, even on Sundays or Holidays or as called for by the school.
For reason, Termination of employment due to violations on School/College policy in case/s where you did
not meet the standard of the School/College for the position or resignation, you must comply but not limited to the
following:
Failure of the employee to comply, it is the right of the School/College to perform legal action to the
employee and recover damages which includes but not limited to:
o Legal fees
o Attorneys fee
o Legal filing fees
3. PERFORMANCE OF DUTIES
The Employee agrees to devote his best efforts, energies and abilities and his full time, skill and attention to
the business and affairs of the School/College, its subsidiaries and other affiliates and shall perform the duties and carry
out the responsibilities assigned to him, to the best of his ability and in a diligent, trustworthy and efficient manner.
4. JOB DESCRIPTION
During the period of engagement with ST. DOMINIC SAVIO COLLEGE, the employee must perform but
not limited to the following duties and responsibilities or Job Description:

Clean building floors by sweeping, mopping, scrubbing, or vacuuming them.
Gather and empty trash.
Service, clean, and supply restrooms.
Follow procedures for the use of chemical cleaners and power equipment, in order to prevent damage to floors
and fixtures.
Monitor building security and safety by performing such tasks as locking doors after operating hours and
checking electrical appliance use to ensure that hazards are not created.
Move heavy furniture, equipment, and supplies, either manually or by using hand trucks.
Notify managers concerning the need for major repairs or additions to building operating systems.
Requisition supplies and equipment needed for cleaning and maintenance duties.
Spray insecticides and fumigants to prevent insect and rodent infestation.
Clean and polish furniture and fixtures.
Clean windows, glass partitions, and mirrors, using soapy water or other cleaners, sponges, and squeegees.
Dust furniture, walls, machines, and equipment.
Make adjustments and minor repairs to cooling, plumbing, and electrical systems.
Mix water and detergents or acids in containers to prepare cleaning solutions, according to specifications.
Clean laboratory rooms and equipment.
Other job that may be assigned by the management.
5. COMPENSATION
Subject to the following provisions of this Agreement, during the Employment Period the Employee shall be
compensated for his services as follows:
Salary: 8,000.00 pesos / month payable every 7th and 22nd of each month
6. PERFORMANCE EVALUATION
The Human Resources as well as the immediate superior shall review the employee performance on a regular
basis. Adjustments to Base Salary, Bonus, or other compensation, if any, and any additional benefit or Awards shall be
made by the President / CEO in its sole and absolute discretion.
7. CONFIDENTIALITY
During and after the Employment Period, the Employee will not divulge or appropriate to his own use or to
the use of others, in competition with the School/College, any secret or confidential information or knowledge pertaining
to the business of the School/College, or of any of its subsidiaries, obtained by him in any way while he was employed
by the School/College.
In the event the employee has been terminated or resigned, all files, supplies, equipment, etc. that was assigned
must be faithfully return to HR department and/or Admin Department in full without securing any on his/her own
possession. Any violations, the School/College has right to file legal action in accordance with the law against the
employee and demand penalty to employee depending on the weight or amount stolen or impose civil damages of
100,000.00 pesos whichever is higher.
If the Employee is suspended and/or temporarily prohibited from participating in the conduct of the
School/College affairs, the company obligations under the Agreement shall likewise be suspended.
If the School/College suspects that the Employee is guilty of the conduct which may, if proved, justify his
dismissal, or has committed a breach of any of the material terms of the Agreement, the School/College may, without
prejudice to its right of summary dismissal and without giving rise to any claim for damages or otherwise against it,
suspend the Employee in his capacity as an employee, during which period the Employee shall not be entitled to work
for the School/College and shall not represent that he does.
9. LEGAL CONTROVERSY
In case any legal controversy will arise out of the performance of your duties and responsibilities, and as
employee of SDSC, or other acts of omissions you have done, and all cases including those criminal in nature must
be filed in the proper Courts of Quezon City Only, TO EXCLUSION OF ALL OTHER COURTS, any provision
of the Law to the contrary notwithstanding;

to be executed in its name and on its behalf, all as of the day and year first above written.

Known to me to be same persons who executed the foregoing instrument and they acknowledged to me that the same are their
voluntary acts and deeds as well as the School/College herein represented.
This CONTRACT consisting of THREE (3) pages, on which this Acknowledgement is written has been signed on the left margin
of each and every page and at the bottom of this instrument by the Parties and their respective witnesses.

APPOINTMENT
MR. HERMIE P. ALCOY
You are hereby notified of your appointment as Probationary- Maintenance Staff under
General Services in charge of maintaining the cleanliness, peace and orderliness of all
classrooms and School Campus of St. Dominic Savio College.
This appointment is effective for the period of one school year (S.Y.), specifically for S.Y.
2019-2020, to commence on ________________ up to _________________, unless expressly
pre-terminated by either party for valid cause and after due process. This appointment is renewable
depending on the job performance, attitude and behavior you have shown during the term of
appointment, as well as the number of students/enrolment and other factors as indicated in the
attached terms and conditions.
It is understood that this appointment is subject to aforesaid attached terms and conditions,
and all other policies, rules and regulations of St. Dominic Savio College which you are required
to read and fully comprehend.

I hereby accept the above-mentioned appointment and agree to abide by all policies and
regulations pertaining to my appointment as Maintenance Staff under General Services in
charge of maintaining the cleanliness, peace and orderliness of all classrooms and School
Campus.
