AFFIDAVIT OF SUPPORT AND CONSENT

I, MA. DULCE AMOR L. JUGUILON, the wife of DON FRANCIS B. JUGUILON and the mother of the
following children:

CARLOS MIGUEL L. JUGUILON;


SARI DOMINIQUE THERESE L. JUGUILON;
JOAQUIN RAFAEL L. JUGUILON; and
MAYUMI L. JUGUILON

citizens of the PHILIPPINES, of legal ages, and residents of SANTIAGO CITY, ISABELA, after having been
duly sworn to in accordance with law, do hereby depose and state:

1. That I am giving full support to apply for passports with the Office of Consular Affairs of the
Philippine Department of Foreign Affairs and to apply for appropriate visas with the SOUTH
KOREAN Embassy in Manila on behalf of my husband DON FRANCIS B. JUGUILON and our
children CARLOS MIGUEL L. JUGUILON, SARI DOMINIQUE THERESE L. JUGUILON, JOAQUIN
RAFAEL L. JUGUILON, and MAYUMI L. JUGUILON

2. That I am giving full consent for my children to travel from the PHILIPPINES to SOUTH KOREA
and accompanied by me and my husband;

3. That I am giving consent and support to procure tickets for our travel;

4. That I am willing and able to maintain and support my husband and my children financially and
guarantee that they will not become a public charge or burden during their stay in the SOUTH
KOREA.
