JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, ROLANDO M. ACMA and AMADOR MARMARA, both of


legal age, Filipino, married, and a resident of Victors Subdivision, Brgy.
Communal, Buhangin, Davao City, respectively, after having been sworn in
accordance with law, hereby depose and say that:

1. We personally know the late RENE BASTES LOMBRES, a resident


of Brgy. Ising, Carmen, Davao del Norte being of acquaintance for
a long period of time;

2. We know for a fact that the person by the name of RENE BASTES
LOMBRES as appearing in his death certificate and the name
RENY BASTES LOMBRES and RENATO BASTES LUMBRES as
appearing in the SSS records refers to one and the same person
and that his true and correct name is RENY BASTES LOMBRES;

3. We are not in any way related by affinity or consanguinity to the


above mentioned person, nor do we have any pecuniary interest in
this matter whatsoever;

4. We are executing this affidavit to affirm and confirm the foregoing


facts and for whatever legal purpose this may serve best.
