Risk Assessment Port Side Lifeboat Engine Starter Failure

The vessel Sea-land Intrepid notified its flag state and class that the port side lifeboat engine was unable to start on battery during weekly testing. Attempts to repair it onboard were unsuccessful. The engine can still be started with the spring starter. Spare parts have been ordered and will be installed at Tanjung Pelepas on August 28th. Risks are mitigated by the operational status of other lifeboats and liferafts, crew awareness, and temporary crew assignments until repairs are complete.

Port Side Lifeboat Engine Unable to Start on battery

Details of Communication with Service Companies

During weekly testing on lifeboat engines, it was discovered that port side lifeboat engine was unable to start on battery.
On board attempts to repairs were unsuccessful without obtaining spares.
Presently the engine can be started with the spring starter.
Due to uncertainty in obtaining spare parts during current coast, required spare parts have been ordered for vessel call at Tanjung Pelepas on 28th
August 2015 as an urgent requisition.

Risk Assessment & Mitigation (to be amended as applicable)


RISK ASSESSMENT :
1. Inability to start port Lifeboat Engine on battery due to equipment failure.
RISK MIGITATION MEASURES :
1. Vessel ensures the other Lifeboat and liferafts are properly maintained and are ready for use.
2. All crewmembers are made fully aware of the defective equipment.
3. The affected lifeboat engine could still be started by secondary method if required in an emergency.
4. All crew from the affected L/B can be temporarily assigned to the oprational L/B, as deemed required.
5. Starting procedure by secondary method is clearly posted in affected lifeboat.
6. Lifeboat provided with two Oars.
7. Entries to vessel’s log book are made and maintained properly.
8. Drill to be conducted with the crew and logged down accordingly.
9. Port State will be notified prior arrival about the deficiency and the action plan to rectify it.
10. Master confirms that the above Risk assessment has been carried out.
11. Master confirms all other equipment and machinery is fully operational.

VESSEL'S SCHEDULE

Expected Service Port and Date : Tanjung Pelepas 28 August 2015 1300LT

VESSEL WILL UPDATE THE FLEET GROUP ON STATUS OF REPAIRS WHEN NOT COMPLETED. ONCE COMPLETED THE
FLAG/CLASS SHALL BE NOTIFIED DIRECTLY AND REQUESTED TO ISSUE A FULL TERM CERTIFICATE.
