EMPLOYEE RECOGNITION AWARD NOMINATION

Objective:

 To motivate employees
 To create environment for fair competition among employees
 To retain the best employees available in the hospital

Nominations may be submitted by any department in charge of the (SMH) on the Nomination
Form.SMH Employee Recognition Award (ERA) is awarded monthly to three employee who
demonstrates a high level of job performance, quality customer service and adherence to SMH
mission and values.

Employee Recognition Award:

Employee selected for Employee Recognition Award will receive a Recognition Award
Certificate by the MD and will also receive a cash award.

Eligible Criteria:

 Job Performance: Employee consistently goes above and beyond what is required for a
particular job.

 Promotion of Safety and Quality: Innovative idea created by employee to improve safety
and quality.

 Positive Attitude: Employee must demonstrate a positive and dedicated attitude.

 Leadership/Initiative: Employee must possess leadership/initiative qualities, with at least


one specific example cited on nomination form.

 Dependability/Punctuality: Employee must be in good standing with maintaining SMH


HR policy on time and attendance.

Employee must meet the following requirements:

 Employee completed minimum three month of service is eligible for ERA.

 No department in charge can be selected.

 Employee cannot have received the Employee of the Month recognition more than once
in a year period.

 Employee must not have had disciplinary action by HR within the last 3 years.

Employee of Month Procedure/Process:

 Forms will also be available in the HR department at beginning of each month.


Department in charge are requested to collect the forms.

 Completed forms will be sent to the HR department by 15th of each month. Department in
charge can nominate maximum of two employees in a month.

 Each nomination will be screened by HR to ensure the criteria are met.

 Final selection of the Employee Recognition Award will be determined by a committee


formed by the management.

 If there is no nominee for a given month, there will be no Employee Recognition Award
for that given month.
