A RESOLUTION REPLACING INACTIVE MEMBERS OF THE ASSOCIATION

WHEREAS, the NAME OF THE SLPA is a duly organized association consisting of (total number of members) during
its inception with a Constitution and By-Laws (CBL) ratified and adopted last Date of the CBL’s ratification;

WHEREAS, the Association has the authority to replace its inactive members and allow the inclusion of new members
in accordance with its CBL and the applicable guidelines and regulations of the Sustainable Livelihood Program (SLP)
of the Department of Social Welfare and Development (DSWD);

WHEREAS, the following members of the association are currently inactive, uninterested and unwilling to continue
participating in the activities of the association:

WHEREAS, there is a need to replace the above-mentioned members for the purpose of receiving livelihood assistance
grants as provided in the guidelines of the SLP;

WHEREAS, upon proper assessment and review of the application for membership to the Association, the following
are considered as eligible to be members of the association and replacements of the currently inactive members:

NOW THEREFORE, BE IT RESOLVED, that the above-mention inactive members be removed from the membership
roll of the Association;

RESOLVED FURTHER, that the above-mentioned applicants assessed and reviewed as eligible be included as
members of the association;

RESOLVE FINALLY, that a copy of this Resolution will be furnished to DSWD Field Office VI.
