REQUIREMENTS FOR ECC AMENDMENT – CHANGE OF NAME/TRANSFER OF OWNERSHIP


1. Transmittal letter addressed to the Regional Director (Engr. Wilson L. Trajeco)

2. Photocopy of previous issued ECC

3. Any Legal Document as Proof of “Change of Name”/Transfer or Waiver of Rights

a. SEC/DTI Registration

b. Board Resolution

4. EMB Compliances (ECC, WDP, POA, HWG ID, CCO and Latest SMR –whichever is applicable)

5. Special Power of Attorney, if applicable

6. Payment: Php 1000.00
