Temperature Record Sheet
(Adapted from EDB template “Guidelines on Prevention of Communicable Diseases in School”)

1. Parents/Guardians are advised to take your child’s body temperature before sending her to
school. If your child has a fever, she should seek a doctor’s attention promptly. She should take
sick leave and rest at home. She should not attend school if she is on medication for fever.
(Different reference values for normal body temperature may be adopted for different measuring methods and
instruments. Always refer to inserts and instructions that come with the thermometer. For screening purpose,
temperature above the reference range quoted below will be considered as significant and one should consult a
doctor for suspected fever.)
