Because you live outside of the United Kingdom (UK) and receive a UK State Pension
you must now send us proof that we should still be paying you. You must fill in and
return the Life Certificate sent with this form. It must be witnessed by one of those
persons shown over the page.
It is important that we receive your reply within 8 weeks from the date of this form.
If we do not receive your completed Life Certificate then I regret that we will have
to stop your money until the Life Certificate is received.

What is a Life Certificate?
The Life Certificate is one of the measures for the Department for Work and Pensions to
obtain proof that a person living outside of the UK is still entitled to be paid a UK State
Pension. Please read the notes CF-N-701, sent with this form, which tell you what
changes you must tell us about.

What must I do with the Life Certificate?


You must read all the information sent with the Life Certificate. You must contact one of
the approved agencies/institutions, sign the Life Certificate in front of the witness and
show them your identity document. The official at the agency/institution you use will
then sign and stamp the Life Certificate. You must then return the completed Life
Certificate and use the reply envelope sent with this form.
The Life Certificate will be checked after we receive it at the International Pension
Centre.

Who can sign the Life Certificate?
The person who signs the Life Certificate should not be related to you by birth or
marriage, not live at the same address as you and should be a professional person or a
person of good standing in the community.
Examples of the type of person that would be suitable include:

• bank/building society official or accountant


• barrister, solicitor or any other person allowed to manage oaths in the country
where the declaration is made
• doctor, dentist, physiotherapist or pharmacist
• government official or local Mayor
• the equivalent of a magistrate, Justice of the Peace or member of the local police
force
• minister of religion
• care home/nursing home manager
If you are unsure then please contact us at once as the Life Certificate will be returned
to you to be signed by someone else if the person you have chosen is not of sufficient
standing.

Proof of Identity
You must show an identity document, which must bear your photograph, to the person
witnessing your declaration. Examples of these include:

• Passport
• National Identity Card
• Driving License
