PETITION

COMES NOW, the undersigned Petitioner in the above


entitled matter and unto this Honorable Court most
respectfully alleges:

1. She is of legal age, Filipino, single, born on


_____________ at the City of Manila, Philippines, a resident of
_________________, Quezon City for the past twenty-seven
(27) years, and with office address at ___________________,
Philippines with telephone number ___________;

2. She is an attorney-at-law, a member of the Philippine


Bar under Attorney’s Roll No. ______and is duly authorized
to practice law under the laws of the Republic of the
Philippines as supported by a photocopy of the Certificate of
Title/ Membership in the Philippine Bar issued by the Office
of the Bar Confidant, Supreme Court of the Philippines, a
copy of which is attached hereto and made an integral part
hereof as Annex “A”;

3. She has paid her Professional Tax for the year 2016
under Official Receipt No. __________ dated ___________ at
Makati City, a copy of which is attached hereto and made an
integral part hereof as Annex “B”;

4. She is a registered member of the Integrated Bar of


the Philippines and has paid her annual membership fee as
evidenced by Official Receipt No. __________ dated
___________, a photocopy of which is attached hereto and
made an integral part hereof as Annex “C”;

5. She has not applied for any notarial commission with


any judicial, quasi-judicial or administrative body;

6. She undertakes to submit all his notarial books and


acknowledged documents after the expiration of his
commission as provided for by law;

7. She possesses all the qualifications and none of the


disqualification for the said office as Notary Public and have
read, understood and is willing to abide with the 2004 Rules
on Notarial Practice (A.M. No. 02-08-13) which took effect
on 01 August 2004;

8. She is of good moral character, has never been


accused or convicted of a crime involving moral turpitude
and has likewise not been a respondent in any
administrative case in connection with his present
profession;

9. She is attaching herewith the sample print of her


notarial or official seal prepared in accordance with Sec. 2,

Rule 7 of the 2004 Rules on Notarial Practice to be
approved by the Honorable Court attached as Annex “E”;

10. He is likewise attaching herewith his three (3) passport


size colored photograph with signature at the bottom as
Annex “F”;

11. Also attached herewith and as required by said 2004


Rules on Notarial Practice are the following documents:
a. Specimen signatures of the herein petitioner,
as Annex “G”;
b. Certification from the Integrated Bar of the
Philippines on Bar Compliance, as Annex
“H”; and,
c. Certification from the Bar Confidant of the
Supreme Court, as Annex “I”;

12. That attached is the official receipt showing proof


of payment for the filing of the petition as required by
the said 2004 Rules on Notarial Practice as Annex “J”;

WHEREFORE, it is most respectfully prayed that:

a. Petitioner’s appointment as NOTARY PUBLIC


for and in the National Capital Judicial Region
covering the City of Makati be issued from
years _____ to _____;

b. Petitioner’s notarial seal impression be


approved, and a certificate of authorization to
purchase notarial seal as provided for under

Sec. ___, Rule ____ of the 200___ Rules of
Notarial Practice be allowed;

c. and the certificate of authority for a Notarial


Act as provided under Sec. __, Rule ___ of said
rules be issued;

Petitioner further prays for such other reliefs and

agency or quasi-judicial body; and,

5. Should it come to my knowledge that a similar action


or proceeding has been filed or pending before the
Supreme Court, the Court of Appeals, the different
divisions thereof, or any other court, tribunal, agency,
or quasi-judicial agency, I hereby undertake to notify
the Court or tribunal taking cognizance of the above
captioned matter of such fact within five (5) days
from receipt of such knowledge.
