• If you are eligible to graduate with a diploma, you must complete and submit this form to The Royal Conservatory by mail or fax no later than
Monday, September 8, 2014. Submit by fax to 416.408.3151 or scan/email to [email protected].
• Complete a separate form for each diploma for which you qualify for graduation.
• A formal convocation invitation will be emailed to you in October 2014 for graduation ceremonies.
• Graduates are responsible for providing a valid email address, mailing address, and correct spelling of their name for the Diploma; a reprint fee
of $80 will be charged for replacement Diplomas. Diplomas damaged in the mail will be replaced free of charge.
