AUTHORITY TO DEDUCT FORM

I, ____________________, hereby authorize my Employer ______________ thru


People Management Department to deduct from my salaries any loan or advances
in the sum of ₱__________, in _____ installment payments done in every payroll
release beginning ________ and ending ________ until the amount of ₱_________
has been fully paid.

In the event that my employment ends for any reason before the final deduction is
made, the entire balance may be deducted from my final pay.

In the event further that my salary is not enough to cover the said deduction, I
hereby authorize the People Management Department to deduct from my Mid-year
or 13th month pay and thus waive my right to collect as such.

I am authorizing this voluntary deduction as specified in Art. 113, Section 10 of


the Labor Code of the Philippines.
