SURVEY NOTES, LIMITATIONS & VALIDITY

This survey was performed to assess the suitability of the vessel for use on the project as described in
Section 1.3. Should the vessel be employed on other activities prior to mobilizing for the project, another
survey may be required to confirm continued suitability.

Structural inspection was limited to those areas readily and safely accessible at the time of survey.
Detailed evaluation of the structural condition of the vessel (such as thickness gauging, non-destructive
examination or Class compliance) was not performed. The report provides a general indication of the
overall structural condition/integrity only.

Except where stated, the survey did not include full function testing of equipment. Whilst the report
indicates the perceived acceptability of equipment in general, it should not be taken as a guarantee that
equipment will perform adequately under operational conditions.

Suitability of the vessel may be subject to satisfactory completion of recommendations given by this
office. Items requiring further attention before final approval for project use are detailed in Section 1.14.

Descriptive terms are used in this report with definitions as follows:

“SATISFACTORY”: A piece of equipment, system or procedure meets a satisfactory


industry standard and/or regulatory compliance and/or LOC guidelines
and is suitable for its intended purpose;

“GOOD”: A piece of equipment, system or procedure conspicuously exceeds the


“SATISFACTORY” standard;

“UNSATISFACTORY”: A piece of equipment, system or procedure does not meet the


“SATISFACTORY” standard due to its inherent design, or type, or due
to a defect requiring repair, modification or other work to achieve a
“SATISFACTORY” standard.

SAFETY AWARENESS

It is important to ensure that a safe environment exists for Surveyors whilst conducting surveys and
inspections. Surveyors are instructed to complete a Dynamic Risk Assessment using the LOC “STAR
DRA” forms for personnel transfers, enclosed space entries or any other evolution that they deem to
warrant a Dynamic Risk Assessment.

Completed “STAR DRA” forms (Part 1 and if applicable 2, 3 and 4) will be retained by the appropriate
LOC Office Safety Officer.

SUMMARY CONCLUSION:
1.13 Suitability: The vessel will be considered suitable for its intended role on the
project on completion of the recommendations listed in Section 1.14, below.
1.14 Recommendations:
Nil.
1.15 Observations:
1) No formal monthly safety meetings were being carried out or logged on board. This
is an area which could be improved in the future (Refer to 9.18).
2) The vessel has an inoperable and sealed off Oily Water Separator (OWS) in the
engine room. There is a certificate from the Classification society acknowledging
the same. All bilge effluent is discharged ashore – Oil Record Book entries and
certificates were sighted regarding the discharge of the bilges to shore facilities.
(Refer to 6.11).
3) Defect reports – part of the HSEQ system - were not sighted. (Refer to 9.7).
1.16 General remarks:
“WIND EXPRESS” appeared to be a well-run vessel with adequate safe procedures
in place and there was ample evidence of these procedures being followed and
recorded.

The vessel was built in 1972 at Waterhuizen, Netherlands and undertook major
conversion in 1999. She is a Diving Support Vessel with 4 point mooring system fitted.

Diving support modules are fitted on board and Mr Derek Boyce (Prodive's IMCA
auditor) was in attendance carrying out an IMCA dive audit aboard the vessel.

The vessel, generally used as an offshore/diving support vessel, is classed by DNV


and flies a Saint Vincent and the Grenadines flag.

No outstanding Condition of Class was noted.

The vessel’s certificates were found in order and up-to-date.

The bridge layout is quite old but new/upgraded equipment has been fitted over the
years. The deck is a bit cluttered due to the diving module and the accommodation
containers for the diver fitted. General condition of the deck is good and there is
evidence of regular maintenance being carried out.

The winches and wires of the 4 point mooring system anchors were in good condition.

The vessel is provided with twin CPP propellers and bow thrusters all reported to be
in good working condition. Power is generated through 6 diesel generators.

Overall the engine room was found in a clean and tidy condition but is not provided
with any fixed fire protection system. However, adequate numbers of fire
extinguishers are in place.

The crew was cooperative throughout the survey.
