SCOPE OF WORK
TABLE OF CONTENTS
1.0 GENERAL
1.1 This Section is to provide CONTRACTOR a general description of the WORK and to outline
CONTRACTOR’s responsibilities for performing the WORK efficiently, safely and no harm to
people.
1.2 The CONTRACTOR shall be deemed to have agreed to comply with all technical requirements
and specifications as detailed in this CONTRACT and relevant COMPANY Manual, Guideline
and Procedures.
1.3 The CONTRACTOR shall provide all the Marine VESSEL, MANPOWER and EQUIPMENT,
consumables, resources, MATERIALS, facilities and additional support services for execution of
the WORK.
1.4 As for base case, CONTRACTOR shall provide all requirements as stipulated in Appendix 1 -
Provision By CONTRACTOR and Section VII - TECHNICAL INFORMATION and/or in relevant
part of this CONTRACT.
1.5 The CONTRACTOR shall be responsible to administer the MANPOWER, Marine VESSEL,
WORKSITE, facilities, EQUIPMENT, other resources and either owned, rented or chartered by
CONTRACTOR and its SUBCONTRACTOR in relation to this CONTRACT.
1.6 The CONTRACTOR is expected to work as a team, proactive, create proper working
environment, optimize the resources and plan effectively within CONTRACTOR organization
and with COMPANY.
1.7 COMPANY has established a base load WORKBARGE and WORKBOAT utilization for FIRM
COMMITMENT and Call-Out basis as follows;
a. FIRM COMMITMENT - COMPANY guarantees cumulative utilization of one (1)
WORKBARGE 3A and one (1) WORKBARGE 3B for 270 days a calendar year for 2013
until 2015. For FIRM COMMITMENT WORKBARGE from 2016 and 2017, a notice will be
given six (6) months prior to commencement of the 2016’s WORK. Provisions of FIRM
COMMITMENT WORKBARGE for this CONTRACT are as stipulated in Appendix 1 –
Provision By CONTRACTOR.
In the event that the award of the contract is delay until 2013, the number of COMPANY
guarantee utilization per calendar year for 2013 shall be reduce accordingly, counting
from the date the WORKBARGE is ON-HIRED until 1st November 2013 e.g. if the contract
is awarded on 31st Jan 2013, WORKBARGE is ON-HIRED on 1st April 2013, period of
COMPANY guarantee utilization per calendar year will be 215 days only (i.e. counting
from 1st April 2013 until 1st November 2013).
b. Call-Out – The base load for WORKBARGE and WORKBOAT on Call-Out Basis is three (3)
WORKBARGE 3B, two (2) WORKBOAT 4A and two (2) WORKBOAT 4B.
COMPANY will not guarantee any utilization days or guarantee continuity of WORK for
Call-Out VESSEL, it will be on ‘as and when’ required basis depending on operational
requirements. Call-Out VESSEL for this CONTRACT are as stipulated in Appendix 1 –
Provision By CONTRACTOR.
1.8 The CONTRACTOR shall mobilize a team of Project Management Team (PMT) member for the
CONTRACT. The PMT comprises team of personnel for HSSE Team, Marine Team, Workpack
Team, Operation Team and QA/QC Team. COMPANY reserves the right to interview the
proposed personnel prior to their mobilization.
1.9 Not Used.
1.10 Other personnel or team member not included in PMT member are deemed workforce required
by CONTRACTOR to complement the PMT and their cost shall be under CONTRACTOR’s own
account.

1.11 COMPANY has established Firm and Call-Out Personnel enable the CONTRACTOR to execute
the CONTRACT as follows;
a. Firm Personnel – CONTRACTOR personnel to be mobilized and maintained throughout the
CONTRACT period.
b. Call-Out Personnel – Additional personnel deem necessary to enhance CONTRACTOR
performance or ‘as and when’ basis depending on operational requirement or as requested
by COMPANY.
1.12 Provisions of Firm and Call-Out Personnel for this CONTRACT are as stipulated in Appendix 1 –
Provision By CONTRACTOR.
1.13 COMPANY reserves the right at its sole discretion to review the requirement, add or delete the
Firm position stated in the PMT by giving one (1) month advance notice in writing.
CONTRACTOR is responsible to act to the requirement as stated in the letter.
1.14 The CONTRACTOR can only mobilize Call-Out Personnel upon giving explanation of it
requirement and COMPANY reserves the right to reject the request from CONTRACTOR if
deemed not necessary.
1.15 The CONTRACTOR shall be responsible to make available the Onshore Facilities to domicile the
PMT, other personnel as well as to support operation and construction for performance of the
WORK.
1.16 Provision of Onshore Facilities and it associated services which required for CONTRACT as
stipulated in Appendix 1 - Provision By CONTRACTOR and shall be deemed included in Section
III - SCHEDULE OF PRICES.
1.17 CONTRACTOR and SUBCONTRACTOR personnel shall be required to work in the onshore or
offshore WORKSITE as and when required.
1.18 Any deviations to the technical requirements and specifications shall only be accepted if the
COMPANY has given its express written approval for the particular deviation.
1.19 Unless expressly stated otherwise, all costs for the fulfillment of the requirements of this Section
and Section VII - TECHNICAL INFORMATION and other relevant Section in this CONTRACT
hereinafter shall be included in Section III - SCHEDULE OF PRICES.
1.20 The rate in Section III - SCHEDULE OF PRICES shall be deemed inclusive of provisions including
but not limited to all services, facilities, equipment, transportations, documentations,
communications, personnel tools, inspection, testing, accommodation and report as required to
perform the WORK.
1.21 The CONTRACTOR shall be fully responsible for the supervision of the WORK and must take all
reasonable measures to ensure WORK is carried out expeditiously, safely and quality to the
COMPANY standard.
1.22 The CONTRACTOR shall provide qualified engineering personnel, construction personnel and
supporting personnel inclusive their tools and equipment for identifying the scope of WORK,
prepare the work plan and completing all the WORKS including commissioning and start up of
the systems within the agreed cost and schedule.
1.23 All CONTRACTOR and SUBCONTRACTOR’s personnel curriculum vitae (CV) have to be
approved by COMPANY prior to mobilization and be given a unique number. It is mandatory
for CONTRACTOR to mobilize personnel meeting minimum specifications defined in Section VII
- TECHNICAL INFORMATION.
1.24 CONTRACTOR is responsible to evaluate all the CV against Section VII - TECHNICAL
INFORMATION and submit evaluations together with training matrix and all necessary
certificates for COMPANY approval. COMPANY reserves the rights to reject any CV without
assigning any reason if deem that the personnel does not meet COMPANY’s requirement.
1.25 If at any time the COMPANY is of the opinion the one or more of the CONTRACTOR’s
personnel is incompetent, misconduct, misbehaviour, inefficient or is negligent in the discharge

of the duty or is otherwise working or behaving is such ways as to make his presence or
employment under the CONTRACT undesirable, the COMPANY may notify the CONTRACTOR
in writing to that effect and CONTRACTOR shall promptly replace such personnel at no cost to
COMPANY.
1.26 The CONTRACTOR shall ensure that all its personnel have undergone the relevant Mandatory
Training as listed in Section VII – TECHNICAL INFORMATION prior to deploying them in PMT
or deploying them to the WORKSITE. The requirement to attend the Mandatory Training is
compulsory and deemed inclusive in Section III - SCHEDULE OF PRICES.
1.27 CONTRACTOR shall provide all logistic arrangements and transportation for the
CONTRACTOR’s and SUBCONTRACTOR’s personnel to report and deliver to the COMPANY
WORKSITE.
1.28 The COMPANY reserves the right to utilize Marine VESSEL, SUPPORT VESSELS and
EQUIPMENT and its facilities onboard that are on hired from CONTRACTOR for other activities
or to be used by COMPANY’s third (3rd) parties.
1.29 If COMPANY decides to mobilize the CONTRACTOR’s WORKBARGE or WORKBOAT, even
though documents stipulated in Appendix 1 - Provision By CONTRACTOR are not acceptable,
COMPANY reserve the rights to withold 5% of all invoices until all documents are acceptable
and approved.
1.30 There is no exclusiveness of WORK in the CONTRACT. The COMPANY reserves the rights and
shall be at liberty to call other contractors/third parties to carry out the WORK as and when
necessary.

4.3 The CONTRACTOR shall provide EQUIPMENT in accordance with COMPANY’s instructions. All
EQUIPMENT shall be certified, calibrated, tested, pre-commissioned and commissioned onshore
shall again be calibrated, tested pre-commissioned and commissioned offshore, unless
stipulated or specified otherwise and/or shall be issued with certificate of fitness by third (3rd)
party.
4.4 The CONTRACTOR shall be responsible for off loading, receiving, un-crating, custody, quantity
accountability and placing all MATERIALS into their either temporary or final positions.
4.5 The CONTRACTOR shall perform receiving inspection of the MATERIALS, prepare report and
advise COMPANY if the is any damage or discrepancies within forty eight (48) hours after the
arrival of the item.
4.6 Upon completion of the installation of the MATERIALS and other appurtenances in their final
position, the CONTRACTOR shall perform a thorough inspection of the MATERIALS for any
damages and report the findings to COMPANY within twenty (24) hours the inspection has
been completed.
4.7 In the event that the MATERIALS were damaged due to CONTRACTOR’s negligence, the
CONTRACTOR is liable to make good of any damage at their own costs prior to handover to
COMPANY.
4.8 CONTRACTOR may make use of the COMPANY’s equipment on the WORKSITE subjected to
COMPANY'S approval. The CONTRACTOR shall return COMPANY’s equipment in their
original operational conditions. In the event that the equipment was damaged by
CONTRACTOR due to negligence, the CONTRACTOR is liable to make good any damage at
their own costs prior to handover to COMPANY.
4.9 Prior to commencement of the WORK, the CONTRACTOR shall be responsible for providing
and setting up onshore and offshore facilities including site offices, enclosed and secured stores
onshore or on the vessels for project materials , construction equipment, scaffolding and
weather protection as deemed necessary to complete the WORK
4.10 The CONTRACTOR shall be responsible for the removal of all temporary tie-down points, sea
fastenings, water proof wrapping and temporary blinds on the MATERIALS and its
appurtenances. Loose ends of cables, openings in piping/tubing must be adequately protected
until the MATERIALS are installed.
4.11 The CONTRACTOR shall reinstall all minor equipment/fixtures that were removed in order to
facilitate transportation of the MATERIALS or to complete the WORK.
4.12 The CONTRACTOR is responsible to provide Transportation Container as stipulated in
Appendix 1 - Provision By CONTRACTOR for transportation of the CONTRACTOR’s and
COMPANY’s material and equipment.
4.13 The maximum period of offshore duty for all levels of personnel shall be in accordance to
COMPANY’s Offshore Health Management Guideline: HSE.XXX.6003 Rev 1 and/or any
revisions or updates to the mentioned document.
4.14 All construction personnel shall be equipped with their respective personal trade toolkits and
consumables. The Personal toolkit contents for all trades are defined in Section VII - TECHNICAL
INFORMATION. The cost of personnel tool kits and its consumable are deem to be part
personnel daily rate.
4.15 All CONTRACTOR’s MANPOWER shall be equipped with adequate personal protective
clothing (inclusive of fire retardant coverall) and equipment relevant to the type of work being
performed. These clothing and equipment shall be as approved by COMPANY. The cost of
personal protective clothing and equipment, and its consumables (if any), are deemed to be
part of personnel daily or monthly rate. However, if any personnel are required to work at
special designated area such as H2S designated area or mercury area, the personnel shall be
equipped with the right equipment and such equipment shall be reimbursed to CONTRACTOR
in accordance to Section III – SCHEDULES OF PRICES. CONTRACTOR shall also ensure that the
quantity provided is sufficient to cover for all COMPANY personnel as required.

4.16 For any personnel who are tasked to perform work that requires the usage of impact resistant
gloves, then CONTRACTOR shall provides such COMPANY approved gloves to the personnel.
The cost shall be reimbursed to CONTRACTOR in accordance to Section III – SCHEDULES OF
PRICES.

7.4 The COMPANY shall give the CONTRACTOR a prior written notice of thirty (30) days for
interim demobilization /mobilization of the FIRM COMMITMENT and Call-Out WORKBARGE
or WORKBOAT.
7.5 The COMPANY shall give the CONTRACTOR a prior written notice of thirty (30) days for any
mobilization of the Call-Out SUPPORT VESSELS and fourteen (14) days for any demobilization
of Call-Out SUPPORT VESSELS.

9.1 GENERAL
9.1.1 The CONTRACTOR’s PMT are responsible for the CONTRACT. They shall be
accountable for the execution of the WORK, including but not limited implement and
maintain HSSE and QA/QC systems, preparing work planning, supplying
MANPOWER, EQUIPMENT and VESSEL, purchasing and expediting the EQUIPMENT
and MATERIALS, operating Minor Fabrication Yard and Logistic Yard complete with
logistic supports.
9.1.2 The PMT members shall be dedicated for this CONTRACT and shall be based in Miri.
Key PMT members shall be provided with mobile phone and shall be standby for any
emergency contact on twenty four (24) hours basis.
9.1.3 Replacement of Firm PMT are to be approved by COMPANY and all cost incurred
associated with the replacement shall be fully borne by CONTRACTOR.
9.1.4 The roles and responsibility of the PMT is as stipulated in Appendix 2 - Roles and
Responsibilities. The CONTRACTOR is strictly to observe the minimum requirement of
the roles and responsibilities.
9.1.5 Without exception, the PMT shall follow COMPANY onshore working hours throughout
the CONTRACT duration.
9.1.6 CONTRACTOR’s PMT may be required to work outside the normal COMPANY working
hours depending on operational requirements if required.
9.1.7 In the event that CONTRACTOR failed to fill any of the Firm positions of the PMT,
COMPANY have the right to deduct relevant amount in accordance to Section III -
SCHEDULE OF PRICES.
9.1.8 Monthly Rate for PMT shall be compensated to CONTRACTOR for such provision by
rate quoted in Section III - SCHEDULE OF PRICES after all the above requirements had
been fulfilled to the satisfaction of COMPANY.
9.1.9 The COMPANY reserves the right to increase or decrease the number of PMT personnel
in Appendix 1 - Provision By CONTTRACTOR and applicable rates shall be in
accordance to Section III - SCHEDULE OF PRICES.
9.1.10 The COMPANY reserves the rights to deduct from the timesheets of any PMT manhours
spent not related to this CONTRACT.
9.1.11 In the event any PMT member is absent for more than two (2) working days including
taking leave, CONTRACTOR shall notify and seek COMPANY’s approval and provide
suitable relief personnel.
9.1.12 If the CONTRACTOR is unable to provide suitable relief personnel, COMPANY shall
deduct the monthly rate in accordance to Section III - SCHEDULE OF PRICES.
9.1.13 The CONTRACTOR shall provide a dedicated Operation Engineer for each
WORKBARGE and WORKBOAT to oversee the day to day operation of WORKBARGE
and WORKBOAT and the assigned project.
9.1.14 CONTRACTOR shall mobilise one (1) pax of Operation Engineer for each
WORKBARGE or WORKBOAT. CONTRACTOR is to note that the number of Operation
Engineer shall depend on the quantity of WORKBARGE and WORKBOAT mobilized.
The cost of the Operation Engineer is deemed to be part of VESSEL Daily Charter Rate.
9.1.15 If the CONTRACTOR plans to change its Firm Personnel from any Firm Position, then
the CONTRACTOR shall inform the COMPANY one (1) month in advance and seek
COMPANY approval for such change. The CONTRACTOR shall ensure minimum one
(1) month handover period between incoming candidate and outgoing incumbent. Any

9.3.6 The CONTRACTOR shall submit to the COMPANY a written record of such periods,
including a list of WORKBARGE or WORKBOAT, SUPPORT VESSELS involved, within
twenty-four (24) hours of occurrence of each such incident.
9.3.7 The CONTRACTOR shall incorporate the duration of all Standby, Waiting On
Weather, Downtimes, Breakdowns, Un-seaworthiness and as such that affect the
operation of the VESSEL and precisely reflect the conditions in Daily Status Report.
9.3.8 The CONTRACTOR shall submit all reports to COMPANY as stipulated in Section V -
ADMINISTRATION INSTRUCTIONS and/or required by COMPANY’s representatives.
9.3.9 All CONTRACTOR’s report shall be signed by both CONTRACTOR and COMPANY
representatives.
9.3.10 The CONTRACTOR shall be responsible to provide supporting documents as a backup
for payment certificate such as signed timesheets, Daily /Weekly Progress Report, travel
request and all relevant supporting documents.
9.3.11 The COMPANY reserves the right to reject the payment certificates or invoices if
supporting document are incomplete, incorrect or missing.
9.3.12 The CONTRACTOR shall be responsible to ensure that all the invoices submitted to the
COMPANY are accurate and supported by relevant document, so as to reduce process
period of payment certificate or invoices.

9.5.5 The CONTRACTOR shall be responsible to control and minimise all downtimes either
attributed by CONTRACTOR or COMPANY, similarly additional work that involve in
completing the base scope.
9.5.6 Upon completion of the WORK or part of the WORK, the CONTRACTOR will be
reimbursed for the following circumstances;
a. In the event that the CONTRACTOR complete the WORK ahead of approved
Workpack duration, CONTRACTOR shall be given incentive for the day saved
based on the early completion percentages in accordance with Section III -
SCHEDULE OF PRICES.
b. In the event that the CONTRACTOR complete the WORK beyond the approved
Workpack duration, the Daily Charter Rate of the WORKBARGE and/or
WORKBOAT will be reduced based on the percentages in accordance with Section
III - SCHEDULE OF PRICES.
c. Item (a) and (b) above shall be measure based on the scope in the approved
Workpack or ‘base scope’ and exclude additional WORK or site instruction.
9.5.7 For the WORK or part of the WORK delay due to CONTRACTOR's not being able to
mobilise the resources or fail to comply with safety procedures, the lost in productivity
due to this shall be counted as CONTRACTOR's delay and shall be measure as part of
the ‘base scope’ WORK duration.
9.5.8 Any delays due to waiting on weather, waiting on COMPANY's supplied materials
which are on the critical path of the WORK or operation constraint shall be excluded
from the ‘based scope’ WORK duration measurement.

9.6.10 The CONTRACTOR shall assist the COMPANY’s Representatives throughout the
CONTRACT duration to derive improvement initiatives for contract management.
9.6.11 The CONTRACTOR shall execute any approved initiatives by COMPANY where
CONTRACTOR actions are required.
9.6.12 The COMPANY shall carry-out six (6) monthly Business Performance Reviews (BPR) for
the CONTRACT in which the CONTRACTOR shall provide all necessary information
pertaining to project performances and KPIs.
9.6.13 The COMPANY reserves the right to implement any initiatives as to enhance the
productivity and performance of the WORK.
9.9 REPORTS
9.9.1 CONTRACTOR shall prepare and submit reports as detailed in Section V -
ADMINISTRATION INSTRUCTIONS.
9.9.2 Post Project Critique shall be issued immediately after the WORK is executed. The
minimum content of the Post Project Critique is detailed in Section V -
ADMINISTRATION INSTRUCTIONS.

10.1 GENERAL
10.1.1 The CONTRACTOR shall prepare Workpack Development and Project Close Out
Procedure and submit for COMPANY approval. The procedure shall detail out all
activities, process, method, usage of construction norms from attaining the scope of
work until Workpack approval and final close out report.
10.1.2 COMPANY will provide Approved For Construction (AFC) packages, Inspection Report,
Plant Change Proposal (PCP). These are the basis for CONTRACTOR to develop the
Workpack.
10.1.3 The CONTRACTOR shall be responsible to ensure the Workpack is perfectly done by
thoroughly checking, fully scope the activities, carry out work scoping offshore and
meeting with COMPANY to resolve technical issue, correctly used the construction
norms and sequence the activities. CONTRACTOR shall verify line by line of the scope
and endeavour to minimize additional works during the execution phase.
10.1.4 The CONTRACTOR shall be responsible to carry out minor engineering work which
include but not limited to review of the AFC drawings, clash check, shop drawings,
detail drawings, lifting calculation, identify rigging aids, proposed suitable method or
special equipment to COMPANY.
10.1.5 All minor engineering work done by CONTRACTOR shall be approved by COMPANY
prior to implementation at WORKSITE.
10.1.6 When preparing the Lifting Procedure, CONTRACTOR shall comply with COMPANY’s
SMEP Lifting and Hoisting Manual SMEP. EP200802258279.
10.1.7 The CONTRACTOR shall indicate any discrepancies and ambiguities in the AFC
packages to COMPANY in Site Technical Queries (STQ).
10.1.8 The CONTRACTOR shall assess all risks for the critical activities and details Controls
and Recovery Measures required during executions. Critical activities shall include
heavy lifts, vessel entry, electrical power tie-in, hot-works in potential hydrocarbon
environment, working with high pressure, working at height, working in marginal sea
state and working in potential toxic gas environment.
10.1.9 The CONTRACTOR shall prepare the Positive Isolations Plan to render the WORKSITE is
safe for construction activities. The CONTRACTOR shall be responsible for obtaining
approval from the COMPANY’s Offshore Installation Manager (OIM) on Positive
Isolation prior to commencement of construction activities.
10.1.10 The CONTRACTOR shall be responsible to convert AFC drawing (only certain drawings)
into Issue For Construction (IFC) drawing and submitted to COMPANY for approval.
The approved IFC drawing shall be used for minor fabrication as well as to be included
in the Workpack for offshore construction.
10.1.11 The CONTRACTOR shall emphasize on health and safety of the personnel and
equipment during WORK planning, shall highlight all concerns to COMPANY and
include it in the relevant section of the Workpack.
10.1.12 CONTRACTOR’s Workpack team member shall be required to participate in
COMPANY construction review and meeting, such as Implementation Project On Paper
(IPOP), HAZID and constructability review sessions.
10.1.13 CONTRACTOR shall provide hardcopies (min. 8 sets) and a soft copy (in PDF format
and planning package in Primavera, P3 file format) of the approved Workpack and
distribute it to relevant parties as stated in Appendix 1 - Provision By CONTRACTOR.
10.1.14 All cost for producing and distribution of the Workpack to relevant parties is deemed to
be included in Section III - SCHEDULE OF PRICES.

10.2.12 CONTRACTOR shall liaise with COMPANY Lead Commissioning Engineer (LCE) for
preparing above deliverables.
10.2.13 Each Workpack planning shall be aimed and aligned with COMPANY commissioning
system priority and strategy. Construction network shall be linked to commissioning
priorities. Project is only considered complete upon completion of commissioning.
10.2.14 CONTRACTOR shall prepare Workpack in most efficient construction methods and
processes incorporating hazards involved with precautions and recovery measures
inplace with the objective of no harm to People, Assets and Environment. In addition,
CONTRACTOR shall optimize usage of multi skill personnel in Workpack preparation.
10.2.15 The CONTRACTOR shall prepare the draft Workpack for challenges session with
COMPANY’s relevant project team or department. The CONTRACTOR shall actively
participate and present their strategies and planning during the challenge session.
10.2.16 CONTRACTOR shall be responsible for the final planning Workpack. Any change to
the approved Workpack shall be challenged and approved by COMPANY.
10.2.17 The approved Workpack may require to be revised and re-approved due to milestones
change, activities or resource optimization, activities clash with others and/or to meet
COMPANY’s business priority, in this circumstances the CONTRACTOR shall revise the
Workpack and resubmit for approval.
10.2.18 The CONTRACTOR shall explain clearly on all the factors that affected the change of
the Workpack planning in the Post Project Critique (PPC) and Project Close-Out Report.
10.2.19 COMPANY shall recognize the Workpack as approved after COMPANY’s
Representatives and/or Project Engineer have signed off the document.
10.2.20 COMPANY reserves the rights to specify additional or reduction of Offshore
Construction Personnel and EQUIPMENT via Workpack challenge session or as and
when required by COMPANY.
10.2.21 The CONTRACTOR shall prepare Workpack progress report and to submit to
COMPANY on weekly basis. Format of the report as per Section V - ADMINISTRATION
INSTRUCTIONS.
10.2.22 CONTRACTOR shall provide Workpack Preparation Aids as specified in Section VII –
TECHNICAL INFORMATION.

10.5.3 For work delayed due to waiting of weather, COMPANY supplied materials which are
on the critical path of the WORK or operation constraint, these delay shall be excluded
from the firmed duration agreed by the COMPANY.
10.5.4 Any changes in Workpack Planning during implementation shall be controlled and
approved by COMPANY.
11.1 GENERAL
11.1.1 The CONTRACTOR shall provide all EQUIPMENT, MANPOWER and service to carry
out fabrication of the WORK or part of the WORK onshore as instructed by COMPANY.
11.1.2 The CONTRACTOR shall provide a Minor Fabrication Yard and it facility in Labuan in
accordance to Section VII - TECHNICAL INFORMATION for the performance of the
WORK. The CONTRACTOR may appoint an third (3rd) party onshore fabricator as
their SUB-CONTRACTOR.
11.1.3 The CONTRACTOR shall manage the Minor Fabrication Yard including provision of
Supervisor, Surveyor, HSSE personnel, inspection personnel, NDT personnel, Material
Coordinator and other supporting personnel as for smooth operation of the yard.
11.1.4 CONTRACTOR is responsible to implement CONTRACTOR’s PQA Plan, HSSE Plan for
Minor Fabrication Yard and maintain in accordance to COMPANY Standard.
11.1.5 If CONTRACTOR opts to subcontract the Minor Fabrication Yard, the management of
the yard, HSSE and QA/QC shall be under CONTRACTOR direct control and
responsibility.
11.1.6 The fabrication scopes are defined in the AFC drawings, Workpack or COMPANY
instructions.
11.1.7 All construction aids such as shed, props, chokes and stands require to assemble the
structural and piping component in order to complete the WORK are to be supplied by
CONTRACTOR at own cost.
11.1.8 The CONTRACTOR shall provide all services such cranes, forklift, lorry, trailer,
Transportation Container, lifting appliance and tackles to move the MATERIALS inside
the fabrication yard. CONTRACTOR shall also provide similar service for
transportations of COMPANY material from or to Labuan Supply Base.
11.1.9 The CONTRACTOR shall be responsible for the accuracies and quality of all fabricated
items. All costs and time, whether incurred at the onshore, associated with remedial
works or corrective actions that are required as a result of these inaccuracies and poor
quality shall be borne by the CONTRACTOR.
11.1.10 If the faults have to be repaired at offshore, COMPANY reserves the rights to deduct
from the timesheets of the CONTRACTOR’ s personnel and EQUIPMENT utilised for the
repair.
11.1.11 No fixed quantity or continuity of Minor Fabrication WORK is guaranteed by the
COMPANY throughout the CONTRACT period.
11.1.12 The CONTRACTOR shall design and provide all temporary supports and timber chokes
to aid the fabrication of Structural Steelwork and pipework. The CONTRACTOR shall
ensure the structures are stable and suitably elevated during the fabrication.
11.1.13 The CONTRACTOR shall provide a hardened or concreted storage area for storing of
all fabricated items in the Minor Fabrication Yard.

11.1.14 The CONTRACTOR shall inform COMPANY of any discrepancies and ambiguities in
the AFC drawing via Site Technical Queries (STQ). STQs on each project are to be
compiled and issue to offshore together with the Workpack and Inspection reports
11.1.15 The CONTRACTOR shall produce As-Fabricated Drawings or Yard Redline Marked-Up
Drawings. The As-Fabricated Drawings shall mark-up with Redline to identify
modifications or revisions during the pre-fabrication stage.
11.1.16 The CONTRACTOR shall issue three (3) sets of As-Fabricated Drawings to COMPANY
within one (1) week after the completion of pre-fabrication together with the closeout
report for each project.
11.1.17 Provision of Minor Fabrication Yard and all personnel, construction
tools/aids/equipment, mobile or fix machinery, material, consumables, services,
telephone/fax line, email service, monthly expenses and all associated cost shall be
deemed included in fabrication unit rates as per Section III - SCHEDULE OF PRICES.
11.1.18 The CONTRACTOR to prepare Material Take Off (MTO) for the WORK or part of the
WORK and submit to COMPANY Site Representative together with Weekly Progress
Report on weekly basis.
11.1.19 The CONTRACTOR shall provide cars for COMPANY personnel usage. The cars
provided shall comply with Section VII - TECHNICAL INFORMATION.
11.1.20 The COMPANY reserves the right to request for Minor Fabrication service in Kota
Kinabalu or Miri. The rate for the fabrication work shall be in accordance with Section
III - SCHEDULE OF PRICES.

12.1 GENERAL
12.1.1 The CONTRACTOR shall fully responsible for the supervision of the WORK and must
take all reasonable measures to ensure WORK is carried out expeditiously, safely and
to COMPANY approved standard while meeting the project milestones.
12.1.2 The CONTRACTOR shall be responsible to deploy skilled and competent MANPOWER
to carry out onshore and offshore construction WORK, if COMPANY found
CONTRACTOR PERSONNEL to be not competent and slow down the progress of the
WORK, CONTRACTOR is responsible to replace the personnel immediately at
CONTRACTOR’s account.
12.1.3 The CONTRACTOR shall provide Offshore Construction Personnel as stipulated in
Appendix 1 - Provision By CONTRACTOR. Their requirement depends on the quantity
of VESSEL and requirement stipulated in the approved Workpack or as and when
required by COMPANY.
12.1.4 The CONTRACTOR Offshore Construction Personnel shall comply with the roles and
responsibilities as stipulated in Appendix 2 - Roles and Responsibilities herein.
12.1.5 CONTRACTOR offshore personnel shall not normally permit to leave the WORKSITE
until the person’s relief has arrived at site. Waive of this rules may be given at the
discretion of the COMPANY Site Representative.
12.1.6 CONTRACTOR offshore construction personnel shall not be permitted to stay offshore
more than forty five (45) calendar days. If CONTRACTOR intends to demob the
personnel earlier then his or her scheduled tour of duty, a valid reason shall be
provided to COMPANY Site Representative.
12.1.7 The CONTRACTOR shall maintain a pool of CONTRACTOR PERSONNEL and
EQUIPMENT with updated databases and inventories to be submitted to COMPANY on
a monthly basis.
12.1.8 CONTRACTOR shall provide their offshore Construction Personnel with their respective
tradesman or personnel toolkits as specified in Section VII - TECHNICAL
INFORMATION.
12.1.9 COMPANY reserves the rights to reject ineffective CONTRACTOR’s offshore
Construction Personnel, inferior or slower EQUIPMENT, process or method proposed by
CONTRACTOR due to whatever reasons.
12.1.10 COMPANY reserves the rights to reject the times record of any tradesman who has not
been provided with all the required personnel tool or tools that are inferior regardless if
the person has been at the WORKSITE sharing tools with others.
12.1.11 For Site Team exceeding twenty (20) personnel, a dedicated Safety Officer is required
to oversee the HSSE aspects of the team.
12.1.12 Offshore work shifts are to be arranged with a minimum of thirty (30) minute handover
period for all supervisors and foremen.
12.1.13 Mode of transport to offshore location either via COMPANY’s Helicopter or Super Fast
Crew Boat (SFCB). CONTRACTOR is responsible to book a seat with COMPANY’s
representatives for booking of helicopter or SFCB and confirm booking forty eight (48)
hours before departure.
12.1.14 Offshore crew change cycle shall be submitted to COMPANY before mobilization and
updated periodically. CONTRACTOR shall submit crew change list seven (7) days in
advance with details of sign-on/sign-off.
12.1.15 CONTRACTOR shall be responsible to supply and use a positive pressure habitats or
welding habitat for all hot work activities in Zone 2 area or as instructed by COMPANY

Site Representative. The positive pressure habitat size shall be able to accommodate site
requirement and condition.
12.1.16 The CONTRACTOR shall responsible to provide trained and certified Abseiler
personnel by IRATA. The Abseiler personnel shall include their abseiling equipment not
limited to Personal Protective Equipment (PPE), ropes, all necessary abseiling gear and
tools. The abseiling equipment is deemed inclusive in personnel daily rate.
12.1.17 During execution of WORK, the CONTRACTOR shall at all time keep the WORKSITE
and is surrounding safe and clean, as well as any other location the COMPANY puts as
its disposal in good state of order and cleanliness. Rejected or expired EQUIPMENT
and MATERIALS shall be removed from WORKSITE by CONTRACTOR.
12.1.18 On completion of work, CONTRACTOR shall completely removed all debris, scraps,
surplus, rejected material or any temporary equipment from WORKSITE which are not
at disposition of COMPANY.
12.1.19 CONTRACTOR’s and COMPANY’s third (3rd) parties personnel may be working at the
same WORKSITE, and CONTRACTOR’s work or use of certain facilities may be
interfere with, as a result of concurrent activities. In these circumstances, COMPANY
may request CONTRACTOR to reschedule the activities in such manner as it will
minimise interference with the WORK or any parties involved.
12.1.20 The CONTRACTOR shall prepare and update a register of site instructions (SI),
additional work (AW) and Technical Query (TQ) for each project or campaign at
WORKSITE. The register shall be submitted to COMPANY Site Representative on
weekly basis.
12.1.21 The CONTRACTOR shall be responsible for provision of safety campaigns and safety
incentive schemes to encourage the safety culture of CONTRACTOR’s MANPOWER.
Such incentives schemes will be at CONTRACTOR’s own account.
12.1.22 The CONTRACTOR shall be responsible for provision of retention scheme to retain
CONTRACTOR’s MANPOWER to work during prolong festive seasons. Such retention
schemes will be at CONTRACTOR’s own account.

12.3.3 The PTW document is a formal document process use to control the WORK or task. This
document is designed as a mean of communication between CONTRACTOR site
supervisors, COMPANY Asset operator and those who are involved in the WORK.
12.3.4 In principle, the PTW applies to all offshore activities that have potential to adversely
affect the safety of personnel, environment and COMPANY’s asset.
12.3.5 The CONTRACTOR’s PTW applicant shall be fully versed in PTW system and be
competent in applying for PTW.
12.3.6 CONTRACTOR shall employ, maintain and develop competent PTW applicant,
COMPANY at its discretion will compensate all active and certified PTW applicant that
are on offshore duty an additional ten percent (10%) on top of their respective trades
“Daily Rate”. The said ten percent (10%) top up for PTW applicant shall be restricted to
Offshore Construction Personnel category. The maximum number of PTW applicant top
up, COMPANY is willing to paid shall also be restricted to the number of PTW
applicant approved in the respective Workpack.
12.3.7 CONTRACTOR is required to maintain sufficient quantity of PTW applicant for the
WORK. The minimum number of PTW applicant per construction crew are 1:5 ratio,
however for specific task such as hot work, mechanical and commissioning activities the
ratio is 1:2.
12.3.8 The CONTRACTOR shall develop specific task based Job Hazard Analysis (JHA) to
cover all the activities, identify risk and hazard, provide controls or mitigation as to
eliminate the risk or hazard.
12.3.9 The CONTRACTOR shall attach a Job Hazard Analysis when applying for PTW. The
JHA shall be endorsed by the relevant parties prior to submission to the Permit
Authorizer.

shall contain safety features to enable them to be used safely in hazardous area in
accordance with COMPANY’ Manual and Guidelines.
12.4.8 CONTRACTOR shall inspect all mobile equipment in accordance to COMPANY’s
Procedure DIN:TCP.XX.1007 rev. 1 to ensure there are in safe condition, well
maintained, safety features are in place and proper function of emergency cut-out
system.
12.4.9 CONTRACTOR shall provide Mobile Equipment Inspector (MEI) as to inspect all
CONTRACTOR’s mobile equipment at onshore and offshore location.
12.4.10 All CONTRACTOR’s Lifting Equipment including tackles and appliance shall comply
with standard BSEN 12079 -1 or DNV 271 and COMPANY’s Lifting and Hoisting
SMEP EP 2005-0264 and IMP 0.03 Rev 8
12.4.11 CONTRACTOR shall provide Lifting Equipment Focal Point (LEFP) to inspect all onshore
and offshore CONTRATOR’s Lifting Equipment.
12.4.12 All CONTRACTOR’s Lifting Equipment shall be maintained and inspected in
accordance to COMPANY’s Lifting and Hoisting SMEP EP 2005-0264 and IMP 0.03
Rev 8 Lifting Equipment.
12.4.13 All CONTRACTOR’s Lifting Equipment shall be inspected by CONTRACTOR appointed
LEFP every six (6) month and shall be colour coded every three (3) month.
12.4.14 A register of all Lifting Equipment includes certificates and/or 3rd party inspection shall
be maintained, filed and kept at all time onboard WORKBARGE and WORKBOAT or
Logistic Yard at Labuan.
12.4.15 CONTRACTOR shall provide complete set of Nominal Tools and EQUIPMENT including
all necessary accessories and consumables for each WORKBARGE and WORKBOAT
as stipulated in Section VII - TECHNICAL INFORMATION.
12.4.16 The CONTRACTOR supplied tubular scaffolding in accordance with Safe Access and
Scaffolding Procedure: DIN: TCP.XX.1008 Rev 2 and/or any revisions or updates to the
mentioned document and shall comply with OSHA / DOSH requirements as and when
implemented.
12.4.17 The CONTRACTOR shall provide preservation and weather protection to protect all the
EQUIPMENT and MATERIALS. The preservation and weather protection material such
as tarpaulin sheets and canvas shall be deemed included in Section III – SCHEDULE OF
PRICES.
12.4.18 CONTRACTOR shall inspect the EQUIPMENT for completeness in their warehouse by
their Supervisory personnel witnessed by COMPANY’s representatives before dispatch
to WORKSITE.
12.4.19 In event that CONTRACTOR fails to provide such complete set of Special Equipment,
COMPANY reserves the rights to reject the equipment and all technicians associated
with the incomplete equipment shall not be compensated.
12.4.20 CONTRACTOR supplied Positive Pressure Habitat shall comply with COMPANY’s ECS
Habitats Standard ECS-WP-009 REV 1 and SMEP Hot Work Management Practice
Guide Supplementary to UIA Hot Work Management Standard EP200904219690 rev.
2.0. The Positive Pressure Habitat material and it associated items shall be in
accordance to Section VII - TECHNICAL INFORMATION.

12.5.2 The CONTRACTOR shall ensure that no interference is caused, in the execution of the
WORK, to any other operations of the COMPANY.

14.1 GENERAL
14.1.1 The CONTRACTOR shall provide Marine VESSEL as per Section VII - TECHNICAL
INFORMATION and Marine and Catering Personnel as stated in Appendix 1 –
Provision By CONTRACTOR.
14.1.2 The CONTRACTOR supplied WORKBARGE and WORKBOAT shall comply to
PETRONAS Guidelines for Barges Operating Offshore Malaysia” (PGBOOM)
14.1.3 The CONTRACTOR shall maintain a team of overhead Marine personnel to maintain
their Marine VESSELS. The team shall not be less than one (1) Marine Operation
Manager, and two (2) Marine Engineers.
14.1.4 The CONTRACTOR shall operate and maintain the Marine VESSEL in accordance to
legislative and COMPANY’s regulations, requirements, policies, procedures and
practices.
14.1.5 All Marine VESSEL shall posses a valid Oil Company International Marine Forum/
Offshore Vessel Inspection Database. (OCIMF/OVID) inspection report (not more than
12 months old since inspection). The inspection shall be carried out by only accredited
OVID inspector. All the cost associated to OCIMF/OVID inspection shall be borne by
CONTRACTOR.
14.1.6 The CONTRACTOR shall submit a Computerized Planned Maintenance System (PMS)
for COMPANY approval. The CONTRACTOR shall carry out VESSEL maintenance as
per Plan Maintenance System (PMS) to ensure trouble free running of all EQUIPMENT
throughout the CONTRACT period. The PMS shall identify all the critical equipments for
the respective VESSEL type. Spares for these critical equipments shall be made
available on board the VESSEL throughout the Contract period.
14.1.7 All maintenance of the Marine VESSEL shall be programmed to minimize disruption to
COMPANY’s project key milestones. The CONTRACTOR shall provide the access to
COMPANY Representative to verify on the system.
14.1.8 The point or place of mobilization and demobilization for the Marine VESSEL shall be
at Asian Supply Base or Labuan Anchorage or Miri Roads Anchorage or any other
place within Sarawak/Sabah Waters, unless otherwise advised by COMPANY.
14.1.9 The CONTRACTOR shall be responsible for making necessary arrangements for
customs, immigration, health, pilotage, port clearance and declarations for entry into
and out of Malaysian waters and between states throughout the CONTRACT period.
14.1.10 The CONTRACTOR shall provide a pilot boat for berthing and unberthing of
WORKBARGE. The CONTRACTOR shall be responsible to provide berthing equipment
which include but not limited to fenders and gangway.
14.1.11 The CONTRACTOR shall ensure that all necessary actions for clearances into and out of
ports which include customs, excise clearances and settlement are properly planned
and executed ahead of time so as to prevent delays in obtaining these clearances.
14.1.12 COMPANY may at its discretion, utilize the CONTRACTOR’s Marine VESSEL and
SUPPORT VESSELS to support COMPANY’s other work activities.
14.1.13 Copies of valid statutory and Classification Certificates, test certificates and necessary
EQUIPMENT certificates and passports and all necessary information relating to the
Marine VESSEL and EQUIPMENT shall be made available at the time of mobilization,
for COMPANY inspection and shall be kept on board the Marine VESSEL at all times.
A comprehensive list of EQUIPMENT and maintenance requirements for each Marine
VESSEL shall be prepared and kept up to date.

14.1.14 All costs incurred for provision of services, operate, maintenance and inspection of
Marine VESSEL, vessel equipment and SUPPORT VESSELS shall be deemed to be
include in Section III - SCHEDULE OF PRICES.
14.1.15 All maintenance (affecting the execution of the WORK) and dry-docking shall be done
before the VESSEL is ON-HIRE or after the VESSEL is OFF-HIRE or in between interim
demobilization and mobilization, and shall be subjected to COMPANY’s approval.
14.1.16 The CONTRACTOR shall have on-call duty Marine Captain for twenty four (24) hours
throughout the CONTRACT to support offshore Marine operation and to liaise with
COMPANY Representative. The duty Marine Captain shall be provided with duty hand
phone.
14.1.17 CONTRACTOR shall ensure VESSEL operation at offshore and jetty are executed in
most efficient cost effective and safe manner and strictly in accordance to COMPANY
standard.

14.2.9 CONTRACTOR shall provide cold buffet to enable quick snacks during the meal times,
which shall consist of fresh fruits, fresh vegetables, breads, cold meats, cheese, jams,
and salads. The cold buffet shall be served in temperature-controlled facilities.
14.2.10 All catering crew provided by the CONTRACTOR on the Marine VESSEL shall meet
Section VII - TECHNICAL INFORMATION and approved by COMPANY all catering
crew shall be provided with uniforms preferably white shirt with black or blue pants,
safety shoes including hats to maintain hygienic standards as required by the
COMPANY.
14.2.11 All bed linens shall be changed not less than once per week and before new crew come
on board for crew change.
14.2.12 All blankets should undergo dry cleaning or chemical treatment at least once per month
or unless blankets are washable. Dirty clothing/linens should be washed separately
from clean/plain cloth using dedicated washing machine.
14.2.13 CONTRACTOR shall provide full laundry facilities on board for cleaning of laundry for
all personnel on board. All washing machines and dryers provided on board are to be
industrial standard heavy-duty type.
14.2.14 CONTRACTOR shall provide all consumables required for the laundering services.
14.2.15 At any times, the CONTRACTOR shall conserve food supply on the WORKBARGE
and/or WORKBOAT which can consume for twenty eight (28) days for maximum
Person On Board (POB).
14.2.16 The CONTRACTOR shall never allow food provision to reduce below fourteen (14) days
on the WORKBARGE and/or WORKBOAT at any one time. This is to cater for any
disruption of food supply from Labuan Supply Base.
14.2.17 The list of food provision inside chiller, freezer and dry store shall be made available
on board at all times to allow COMPANY Site Representative to verify the supplies on
the WORKBARGE and/or WORKBOAT.
14.2.18 VESSEL drinking water shall be clear and free from chlorine, bad taste, lead and
harmful contaminants. CONTRACTOR shall provide water filter such Diamond filter or
equivalent and UV steriliser.
14.2.19 The CONTRACTOR shall perform water quality analysis for all the VESSEL on monthly
basis, the analysis shall comply with International Standard for Raw Water Quality
Specification of World Health Organisation (WHO).
14.2.20 As minimum, CONTRACTOR shall perform analysis for Iron, Chloride, Zinc and
Bacteriology (Coliform form and E.Coli).
14.2.21 The source for water analysis shall be taken form Potable water tanks, Water maker
outlet, Galley water tanks and any water outlet inside accommodation.
14.3 REMOTE FUEL MONITORING SYSTEM (FMS) AND CLOSED CIRCUIT TELEVISION (CCTV)
14.3.1 The CONTRACTOR to provide the FMS system as per Section VII - TECHNICAL
INFORMATION. The FMS shall be temper proof, capable to measure fuel consumption
with data of engine RPM, running hours and able to transmit data back to shore at least
four (4) time a day.
14.3.2 All Marine VESSEL shall be equipped with FMS to track the fuel consumption for main
engine, diesel generator and truster.
14.3.3 The same system or separate system shall be capable of measuring and tracking fuel
received from supply vessel and fuel discharge to other Marine VESSEL as well as
construction equipment.

14.3.4 CONTRACTOR shall provide CCTV Surveillance System as stated in Section VII -
TECHNICAL INFORMATION for continuous monitoring of the ship sides, bow, stern,
helideck and engine room.
14.3.5 The CCTV shall be linked to satellite internet system and broadcast live in the web
system able to be viewed by COMPANY at shore.
14.3.6 The minimum requirements of CCTV system are as follows;

14.7.7 COMPANY reserves the rights to reject any CONTRACTOR’s and SUBCONTRACTOR’s
personnel from boarding the Helicopter and SFCB, if his or her mandatory training has
expired or details in database is incomplete or any other reason that might related to
HSSE issues.
14.7.8 CONTRACTOR shall conduct boat transfer and helicopter briefing where applicable,
prior to boarding the SFCB or Helicopter.
14.8 HELIDECK
14.8.1 All WORKBARGE shall have helideck for crew change and emergency evacuation. The
CONTRACTOR’s WORKBARGE helideck shall comply with Section VII - TECHNICAL
INFORMATION.
14.8.2 CONTRACTOR shall be responsible to manage the helideck and its associated
equipment including providing adequate personnel, communication and helideck
equipment for safe landing and takeoff of COMPANY’s Helicopter.
14.8.3 CONTRACTOR shall provide fully trained Helicopter Landing Officers (HLO) and
Helideck Assistance (HDA) and Fire Fighter to assist in embarkations and
disembarkation of passengers and luggage from and helicopter.
14.8.4 CONTRACTOR shall train Deck Foreman to be HLO and Deck Hand to be HDA and
maintain their availability throughout the CONTRACT period.

15.1 GENERAL
15.1.1 The CONTRACTOR shall develop Procurement Procedures, which shall be in line with
COMPANY tendering guidelines and procedures. No procurement shall commence
until this procedure is approved by COMPANY.
15.1.2 Except where it is expressly specified in the purchase order that the MATERIALS or parts
thereof are to be provided by the COMPANY, the CONTRACTOR shall provide all
MATERIALS required in performing and completing the WORK.
15.1.3 The CONTRACTOR warrants that all MATERIALS, EQUIPMENT and services procured
under CONTRACT are fit for intended purposes, of good quality and workmanship,
meet accepted and specified safety standard and in accordance to Section VII -
TECHNICAL INFORMATION or and/or AFC Drawings, Material Take-Off (MTO),
Design Engineering Practice (DEP) and Standard Engineering Specification (SES).
15.1.4 Unless otherwise specified by the COMPANY, all MATERIALS and EQUIPMENT supply
under the CONTRACT shall be new. Where applicable, these MATERIALS shall be
accompanied by document which related to specification, data sheets, manufacturer
and test certificates.
15.1.5 All MATERIALS supplied by the CONTRACTOR for incorporation into the WORK shall
be new, free from defects and fit for their intended purposes. MATERIALS incorporated
into the WORK shall be traceable to certificates. The grades of certificates (A, B, C in
accordance with DIN 50049) shall be determined by the CONTRACTOR and agreed to
by the COMPANY for different types of MATERIALS.
15.1.6 The COMPANY reserves the right to accept or reject any MATERIALS provided by the
CONTRACTOR and demand removal from the WORKSITE of any MATERIALS provided
by the CONTRACTOR found to be defective. Replacement or rectification of defects or
damage of MATERIALS provided by the CONTRACTOR shall be at CONTRACTOR’s
own costs.
15.1.7 The CONTRACTOR shall procure and expedite all CONTRACTOR supplied MATERIALS
for a timely delivery to achieve the project milestones. The CONTRACTOR shall include
Source Inspections for Corrosion Resistant Alloy materials such as Duplex, Super
Duplex and Inconel on Cost-Plus Basis. Positive Material Identification (PMI) report shall
be provided for the Corrosion Resistant Alloy materials.
15.1.8 The CONTRACTOR shall be responsible for the accuracy of the MTO and adequacy of
MATERIALS. The CONTRACTOR is responsible to inform the COMPANY of any
shortages.
15.1.9 Where a Purchase Order is issued on a Unit Rates basis, the CONTRACTOR shall be
compensated based on the actual quantity used/incorporated into the WORK, verified
and approved by COMPANY’s Representatives.
15.1.10 The CONTRACTOR shall be reimbursed for such procurement, at the net invoiced cost,
plus the percentage mark-up contained in Section III - SCHEDULE OF PRICES.
15.1.11 The CONTRACTOR shall not accept MATERIALS without the required documentation
and test certificates. COMPANY reserved the right to reject any and all MATERIALS not
meeting this requirement
15.1.12 The CONTRACTOR shall maintain adequate records of all COMPANY supplied
materials while in their custody. COMPANY reserves the right to recover the full cost of
any losses while in CONTRACTOR’s custody.
15.1.13 The CONTRACTOR shall only procure MATERIALS, EQUIPMENT and services from the
PETRONAS Approved Licensed or Registered Vendors. COMPANY shall issue the

updated Standard Work and Equipment Category (SWEC) lists to the CONTRACTOR
upon request from CONTRACTOR. CONTRACTOR shall invite all vendors listed in the

15.3 SPARE PART AND CONSUMABLES FOR CONTRACTOR’s TOOLS AND EQUIPMENT
15.3.1 The CONTRACTOR shall provide throughout the CONTRACT period, all necessary
supplies, spares, expendables, replacement part that required for continuous
performance of the WORK.
15.3.2 The CONTRACTOR shall provide all consumables for CONTRACTOR and SUB-
CONTRACTOR supplied tools and equipment as defined but not limited to the list in
Section VII - TECHNICAL INFORMATION. The costs of all the spare part and
consumables are deemed inclusive in Section III – SCHEDULE OF PRICES.
15.3.3 If under any circumstances the CONTRACTOR is unable to proceed with the WORK or
part thereof as result of shortage of the aforesaid consumables, the affected
CONTRACTOR personnel and equipment shall be deemed to be OFF-HIRE in
accordance to Section III - SCHEDULE OF PRICES.
15.3.4 The CONTRACTOR shall also provide any item of EQUIPMENT spare part,
consumables, supplies and MATERIALS which are not specifically detailed in Section VII
- TECHNICAL INFORMATION but which are required for normal and safe operation
according to good industrial practice.
15.3.5 CONTRACTOR shall be responsible for maintaining adequate stocks of spare part,
consumables and expendables and for replenishment of CONTRACTOR’s EQUIPMENT
so that off-take can immediately take place without affecting COMPANY’s project
schedules.

22.5 The design for the seafastening shall be supplied by CONTRACTOR on Cost-Plus basis. The
design shall be certified by a third (3rd) party surveyor and submitted to the COMPANY not later
than two (2) weeks prior to load-out
22.6 It is the COMPANY’s intention in most instances to free issue the sea-fastening materials to the
CONTRACTOR, however COMPANY may from time to time request the CONTRACTOR to
procure seafastening materials at cost plus basis.
22.7 The CONTRACTOR shall at its own cost ballast and trim the VESSEL for the safe load-out of the
WORK. Ballasting or de-ballasting of the VESSEL shall be carried out under the supervision of
the tug master.
22.8 The CONTRACTOR shall be fully responsible for load-out and sea-fastening of all Cargoes on
their Marine VESSEL during voyage.

23.3.8 CONTRACTOR shall provide a speakerphone system in the Clinic on board all
WORKBOAT and WORKBARGE. The speakerphone shall be connected to the Satellite
Communication system.
23.3.9 CONTRACTOR shall provide walkie-talkies for Offshore Construction Personnel and
COMPANY’s Representatives use offshore. CONTRACTOR shall provide twelve (12)
units of walkie-talkies for CONTRACTOR use and six (6) units for COMPANY use (on
top of the Nominal Equipment) for each WORKBARGE or WORKBOAT.
23.3.10 CONTRACTOR shall provide a sufficient number of telephones to cater for the
CONTRACTOR and SUBCONTRACTOR personnel onboard WORKBARGE and
WORKBOAT, to call home or call their office.
23.3.11 COMPANY will not compensate any claim on offshore telephone bills /calls, therefore
CONTRACTOR is responsible to monitor and control the usage of telephone onboard
WORKBARGE and WORKBOAT.
23.3.12 CONTRACTOR and SUBCONTRACTOR personnel shall not use COMPANY’s facilities
on board the WORKBARGE, WORKBOAT or the offshore platforms.

3.1 General
All Offshore Construction personnel shall be based offshore. The actual line of reporting
may change depending on the nature of job requirement and organisation structure.
3.10 Mechanic
a. Shall report to the Construction Superintendent or Construction Supervisor
b. Shall perform daily routine check of all construction equipments prior to usage
c. Shall maintain construction equipment and repair if breakdown. Construction
Equipment that requires more than three hours to repair shall be replaced.

4.1 Master
a. Shall report to the Senior Marine Captain and base offshore

c. Shall be fully accountable for the functions, operations, maintenance and safety of the
assigned VESSEL
d. Shall train and supervise the Marine crew in the safe and efficient performance of duties
e. Shall in charge for anchor handling operation and closely monitor weather condition at all
time
f. Shall report to COMPANY Site Representative of vessel and weather condition and status
g. Shall responsible and manage all Marine and catering crew
h. Shall maintain all the vessel documentation are valid and up-to-date
i. Shall be the omnipotent person in all Marine safety and “emergency at sea” related
activities
j. Shall observe and comply with all COMPANY Procedures and Guidelines when within the
500m radius of COMPANY assets
k. Shall responsible for operation of spread vessel coming along side

4.6 Medic

b. Shall provide basic medical treatments and consultation services
c. Shall safe keep and maintain the medical supplies and equipment inside the clinic
d. Shall maintain an up-to-date list of DFAs and to conduct the DFA quarterly skill
maintenance training
e. Shall submit detail medical report following FAC and MTC
f. Shall reinforce the cleanliness and good hygiene of the galleys and living accommodation
with the Camp Boss and submit weekly inspection report to Shell Health Lutong
g. Shall submit weekly and monthly Occupational Health (OH) reports to Shell Health Lutong
h. Shall perform routine health monitoring and implement health promotion for all POB
i. Shall check the validity of the POB’s Safety Passport (Mandatory Trainings and Fitness to
Work) and health declaration (examination if required) of incoming POB
j. Shall communicate with ISOS Alarm Center in the event of medical emergencies
k. Shall communicate with CONTRACTOR’s appointed doctor for medical prescriptions
l. Shall assist COMPANY in compliance to HSSE Control Framework.
4.9 Deck Foreman cum Helideck Landing Officer (HLO) and Boat Landing Officer
a. Shall report to the Chief Officer and base offshore
b. Shall be fully accountable for all rigging activities on the vessel deck
c. Shall lead the Deck ERT team
d. Shall responsible for deck maintenance of the vessel
e. Shall responsible as Boat Landing Officer (BLO)
f. Shall responsible for boat transfer operation and communicate with Radio Operator for
safe operation
g. Shall responsible to ensure all boatlanding equipment are in good condition at all time.
4.10 Deck Hand cum Helideck Deck Assistant (HDA) and Gangway Watcher
a. Shall report to the Deck Foreman and Chief Officer, and base offshore.
b. Shall assist Deck Foreman for deck maintenance of the vessel
c. Shall assist Deck Foreman during boat transfer and as required
d. Shall assist Deck Foreman to ensure that all rigging activities are being carried out in a
safe and effective manner
e. Shall responsible as Gangway Watchers
f. As Gangway Watcher, shall be fully responsible to ensure the safe operations of the
gangway in accordance to the Gangway Operation Guideline
g. On the WORKBARGES; at least two Deck Hands shall be designated as Helicopter Deck
Assistant
h. As Helicopter Deck Assistant, shall assist the HLO to load and unload the baggage on and
from the helicopter

the laundry and living spaces are properly cleaned.
5.3 Cook/Baker

b. Shall assist the Camp Boss to prepare high quality meals in terms of hygiene, varieties and
tastes.
