I, XXX, Filipino, of legal age, residing at Brgy. Balud Del Sur, Gubat,
Sorsogon, after having been sworn in accordance with law hereby depose and
state that:

1. I am the owner of the residential house located at Roxas St., Brgy.


Balud Del Sur, Gubat, Sorsogon;

2. I do hereby declare that I am the lawful owner of the said residential


house, hence, I am applying for the change of the account and billing name for
the above-mentioned address with SORECO II from __________ to herein
affiant, ---------; and

3. I execute this affidavit to establish the truth of the foregoing facts


and for whatever legal intents and purposes this may serve.
