Far Eastern University Institute of Nursing Instructional Script With Actual Findings of Assessing Musculoskeletal

The document provides findings from assessing the musculoskeletal system of a client named Samielle Alexis Garcia. The assessment found that the client showed normal muscle size and tone, full range of motion without pain or limitations across all joints, and full strength graded at 5/5 for all muscles tested in the upper and lower extremities. No abnormalities, deformities, or deviations from normal were observed for the client's muscles, bones, joints, or skeletal structure.

normal full movement against
gravity and against full
resistance.

For the Trapezius, the client

has a Grade 5 for the muscle
strength it has a 100% normal
muscle strength, normal full
movement against gravity and
against minimal resistance.

7. Test muscle strength of For the Deltoid, the client has

upper extremities a Grade 5 for the muscle
strength it has a 100% normal

For the biceps and triceps, the

client has a Grade 5 for the
muscle strength it has a 100%

For the wrist and finger

muscles, the client has a Grade

For the grip strength, the

client has a Grade 5 for the

8. Test muscle strength of For the hip muscles, the client
lower extremities has a Grade 5 for the muscle
strength it has a 100% normal
muscle strength, normal full
movement against gravity and
against minimal resistance.

For the hip abduction and hip

adduction, the client has a
Grade 4 for the muscle
strength it has a 75% normal
muscle strength, normal full
movement against gravity and
against full resistance.

For the Hamstrings and

Quadriceps, the client has a

presence of nodules.
13. Assess joint range of For the flexion, the client can
motion of the head move its neck-pivot joint at
approximately 45° with the
chin resting on the chest.

For the extension, the client

positioned his head vertically
at approximately 0-5° in an
upright position.

For the hyperextension, the

client can move its Neck-Pivot
Joint at approximately 55° in a
flexed upright position.

For the lateral flexion, the

client can move its neck at
approximately 40° to both left
and right side.

For the rotation, the client can

move its Neck-Pivot Joint at
approximately 70° to the left
and to the right.
14. Assess joint range of For the flexion of the client's
motion of body trunk trunk, it has an angle of 90°,
for the extension, it has a 0°
angle and for the
hyperextension, the client has
a 25° angle.

For the lateral flexion, the

client can move side to side at
a 35° angle to each side.

For the rotation, the client can

move at a 30° angle.
15. Assess joint range of For the client's Shoulder-Ball-
motion of upper and-Socket Joint, the flexion
extremities has a 180°, extension with 0°
and hyperextension with 25°.
Abduction with 180° and
adduction with 44°. For the
circumduction, the client can
easily move his arms in a
circular motion. For the
internal and external rotation
of the client, it has both 90°
angles.

For the Elbow-Hinge Joint of

the client, the client's flexion
has a 160° angle and 0°
extension. For the Rotation for
Supination & Pronation, both
have a 90° angle.

For the client's Wrist-

Condyloid Joint, the flexion
has an 80°, the extension with
0°, and hyperextension with
70°. For the radial flexion of
the client, it has a 15° angle,
and 45° for the ulnar flexion.
For the flexion of the fingers, it
has an 85° angle, 0° for the
extension and 25° for the
hyperextension. For the
abduction and adduction of
fingers, the client can move its
hands smoothly.

For the Thumb-Saddle joint,

the client can move his fingers
across the fifth finger, can
move his thumbs laterally, and
can touch the top of each
finger using the thumb.
16. Assess joint range of For the Hip-Ball-and-Socket
motion of lower Joint of the client, the flexion
extremities has an angle of 94°, extension
with 0°, and hyperextension of
20°. For the abduction, it has a
50° while during adduction, it
has a 30° angle. The client can
easily move his legs in a
circular motion. And for the
Knee-Hinge Joint, the client
can move easily his joints
without pain or discomfort.
17. Document findings and
observations.
