Dear Sir:

Good day.

I am Renerio Lamoste, resident of Purok 2-B La Filipina, Tagum City. I am a Construction


Worker (Laborer) for the projects of Engr. John Eduard D. Gallego, resident of Chapter 7-A,
Camiguin, Aplaya, Digos City. I was assigned to his several pending construction projects within
Davao City. The following are the on-going construction projects which I am assigned:

1. Two-Storey Residential House


Owner: Juvy Mabini
Location: Northcrest Subdivision, Cabantian, Davao City

2. Two-Storey Residential House


Owner: Revsan Ethelbert P. Elizalde
Location: Ciudad Verde Subdivision, Maa, Davao City

In relation to the foregoing, may I request from your good office the permission to be allowed
enter to and exit from the said locations so that I may attend to these projects? It is of utmost
importance that I personally oversee these projects because I have to ensure my attendance to
these projects so that I can perform my job in the schedule without delay. I am fully aware of the
present situation. Thus, I also undertake that I will strictly observe the directives of the
authorities in relation to my entry and exit, and to the construction projects as well.
