The SPED’ers C.A.R.E (Concerned Advocates Reaching Out to the Exceptional) officers and
members would like to inform you about our upcoming Kick-Off Party. The said party will be
held on May 22, 2018; 9:00-12:00 pm at Student Center. This aims to give tributes and
recognition to the graduating SPED Majors.

In connection to this, may we ask for your approval to allow us to conduct this Kick-Off Party.
Rest assured that we will abide with the rules and regulations for proper conduct of this activity.
We will provide a parental consent and waiver form for those 2 nd Year and 3rd Year Students who
will be might involve in the said activity, especially the officers of this organization.

respect to the planned activity, duration, location, method of transportation, participants and supervision.

I/We, hereby acknowledge that certain RISK OF INJURY are inherent to participate in learning
activities outside the school. These types of injuries may be minor or serious and may result from one’s
actions, or the actions or inaction of others, or a combination of both.

I/We understand that the Rules and Regulations established for the field trip are designed for the
safety and protection of the participants and hereby undertake to inform my child to abide by these rules
and regulations.

I/We understand that:

1. Minimum level of fitness and health (physical, mental and emotional), is required:
2. Each person has a different capacity for participation; and
3. Any exceptions to full participation are identified on the Child/Student’s Health Form.
