Functional Independence Measure

The document provides a Functional Independence Measure (FIM) assessment for an individual. The assessment evaluates 13 activities of daily living (ADLs) and rates the level of assistance needed on a scale of 1 to 7. The assessment found the individual requires minimal to maximal assistance for some mobility and self-care activities like bathing, dressing, toileting, and transferring, but is independent in other areas like feeding, grooming, bladder/bowel management, and social cognition. Precautions are needed given the assistance required for some activities to prevent falls.
