AFFIDAVIT OF LOSS
I, ERNESTO T. SURIAGA, of legal age, Filipino, with address at Luksuhin
Ibaba, Alfonso Cavite after having been sworn to in accordance with law, hereby
depose and state, that:

1. I am in possession of the following Identification Cards:

-SSS ID card
-Voter’s ID card
-TIN ID card

2. That I used to put said ID cards inside my wallet, however, said


wallet and ID cards was stolen by one unknown person while it was
place inside our Gen. Merchandise Store in Luksuhin Ibaba,
Alfonso Cavite;

3. Despite diligent search and efforts to recover the said wallet and ID
cards the same can no longer be located;

4. I am executing this affidavit not for any unlawful purpose but for
any valid and legal purpose only.

IN TRUTH WHEREOF, I have hereunto affixed my signature this 5th day of


March, 2020 at Alfonso, Cavite.
